This role serves as the primary clinical resource, developing individualized care plans, managing symptoms, coordinating interdisciplinary services, and providing education and emotional support throughout the hospice journey. The RN Case Manager also provides functional supervision and direction to Hospice Aides and LPNs while ensuring quality patient outcomes and regulatory compliance. Key Responsibilities

• Conduct comprehensive patient assessments and develop individualized plans of care. • Coordinate care with physicians, social workers, chaplains, aides, and other team members. • Manage pain and symptoms using evidence-based hospice and palliative care practices. • Provide education and support to patients, families, and caregivers regarding disease progression and end-of-life care. • Monitor patient status and communicate changes promptly to physicians and the interdisciplinary team. • Participate in IDG/IDT meetings and collaborate on patient care planning. • Ensure accurate and timely documentation in the electronic medical record. • Coordinate referrals, recertifications, supplies, and patient care services. • Supervise Hospice Aides and LPNs to ensure quality care delivery. • Promote dignity, comfort, and quality of life for hospice patients and their families. Qualifications Required

• Graduate of an accredited Nursing program. • Current Ohio Registered Nurse (RN) license. • Current CPR certification. • Minimum one (1) year of acute care nursing experience. • Valid driver's license and reliable transportation. • Excellent communication, assessment, and critical thinking skills. Preferred

• Hospice experience. • Home Health experience. • Case management experience. • Knowledge of hospice and palliative care principles. What Makes You Successful
