First Choice Home Health & Hospice – Salt Lake County, UT Part-Time / On-Call Rotation: 1 Week On, 2 Weeks Off Pay: $5/hour while on call + $75–$125 per completed visit
Flexible On-Call Nursing with a Predictable Rotation
First Choice Home Health & Hospice is hiring an experienced Registered Nurse (RN) to provide after-hours support for our home health and hospice patients throughout Salt Lake County.
The current rotation is one week on, followed by two weeks off, making this a great opportunity for an experienced RN who wants to supplement their income without adding another traditional nursing schedule.
You are paid $5 per hour while assigned to on-call availability, plus $75–$125 for each completed patient visit, depending on visit type.
What You'll Do
As an On-Call RN, you'll provide after-hours clinical support to patients and families when needs arise outside normal business hours.
Responsibilities include:
• Respond to after-hours patient and caregiver calls
• Assess patient needs by phone and determine when an in-person visit is appropriate
• Complete home health and hospice nursing visits as needed
• Assess changes in patient condition
• Provide symptom management and nursing interventions according to the plan of care
• Administer medications and treatments as ordered
• Educate and support patients, families, and caregivers
• Communicate changes in condition to the clinical team
• Coordinate with physicians and other providers when needed
• Complete accurate and timely clinical documentation
• Support hospice patients and families during urgent or end-of-life situations
Qualifications
Required:
• Current Registered Nurse (RN) license in good standing
• 2+ years of direct patient-care nursing experience
• Strong nursing assessment and clinical decision-making skills
• Excellent communication skills
• Comfortable working independently in patients' homes
• Valid driver's license and reliable transportation
• Ability to travel throughout Salt Lake County during assigned coverage
• Dependable and responsive during your on-call rotation
Preferred:
• Home health experience
• Hospice experience
• Palliative or end-of-life care experience
• Previous on-call or after-hours nursing experience

Compensation
• $5.00 per hour while assigned to on-call availability
• $75–$125 per completed patient visit, depending on visit type
