Job summary
The Frailty SDEC Clinical Lead is responsible for the operational and clinical leadership of the Frailty SDEC service, ensuring the delivery of safe, effective, person-centred care for older people with frailty. The postholder will provide visible clinical leadership, coordinate multidisciplinary working, support admission avoidance and timely discharge pathways, and ensure the service dleivers high quality care aligned with the Trust Integrated Cre System and National frailty standards
Main duties of the job
We require a motivated, compassionate and enthusiastic leader for our Frailty Same Day Emergency Care service. The service provides timely multidisciplinary assessment and intervention for people living with frailty, supporting admission avoidance, early supported discharge and safe transfer of care.
Proven leadership, excellent interpersonal skills and the ability to influence, support and motivate others are essential to this role.
Provide a focused leadership role to manage staff and activities within Frailty SDEC.
Act as a visible role model and maintain the highest standards of patient care.
Lead and coordinate the nursing team within the service, demonstrating compassionate leadership and effective role modelling.
Work collaboratively with the multidisciplinary team to deliver comprehensive assessment, treatment and discharge planning.
Support admission avoidance and timely discharge using a Home First approach.

The team works closely with acute services, community services, therapists, social care, primary care, specialist teams and voluntary sector colleagues to deliver coordinated, person-centred care. Provide clinical expertise and work as part of a multidisciplinary team.

Qualifications and training Essential
Registered Nurse with current NMC registration Degree in Nursing or equivalent experience Evidence of continuing professional development
Desirable
Leadership or management qualification Frailty, older people's care, urgent care or advanced assessment qualification Previous demonstrable experience at Band 6 or Band 7
Knowledge and experience Essential
Significant post-registration experience. Experience of working with older people or people living with frailty Experience of leadership or management within a clinical setting Experience of multidisciplinary working Evidence of supporting service improvement or change Strong communication and organisational skills
