Are you an experienced Healthcare Support Worker looking for a new challenge in a community setting? Come and join our Community Care Night Service Teams and help us to continue providing high quality and innovative care to the people of Harrogate and Rural area, in the comfort of their own homes.

As a Community Healthcare Support Worker with Harrogate and District Foundation Trust, you will be based in one of our four Community Care Teams. The night service runs between 19.45 and 07.15 every day of the year.

Whilst we are able offer flexible shift patterns, please note that this role is nights only.

Main duties of the job

To assist in assessing people's health and social related needs. This will include:

Assisting in the assessing of peoples' health and social care needs Taking and interpreting physical measurements and samples from named patients as delegated by the team Monitor changes in patients, correctly interpreting their meaning and informing the team when changes might be significant To assist in planning, delivering and evaluating nursing care and rehabilitation programmes to address peoples' health and social needs according to their care plans.

Qualifications Essential

NVQ Level 3 (RQF accredited) in Health and Social care or Care Certificate (NHS England accredited) A full UK driving license to meet service needs

Desirable

First Aid Certificate Other relevant training

Experience Essential

Experience of working within health or social care environment

Knowledge Essential

Clerical and IT skills

Desirable

Understanding of Medicines Management
