Job summary
We are looking for a Band 6 Registered Nurseto join our busy Hospice at Home Team based in Erdington.
As a Band 6 Registered Nurse, you will play a key role in supporting patients with complex end-of-life needs, contributing to the assessment, planning, implementation and evaluation of holistic care for our patients and their carers, ensuring that all care is delivered with compassion, dignity and respect.
Main duties of the job
Provide specialist clinical hands-on care through holistic assessment, planning, implantation and evaluation of care, ensuring the delivery of safe, effective and compassionate person-centred support tailored to the individual needs of the patients and their carers
Work autonomously within professional boundaries, whilst demonstrating advanced clinical rational and decision-making skills to triage, prioritise and coordinate care according to patient need, urgency and complexity
Undertake assessment of patients with palliative and end of life care needs, identifying physical, psychological, social and spiritual needs whist completing appropriate accurate documentation with assistance from patents their families and the members of the multidisciplinary team.
Utilise specialist clinical skills in accordance with professional standards and organisational policies.
What Were Looking For
Level 1 Adult Registered Nurse, with current NMC registration.
Post registration qualification in palliative care, or willingness to undertake one
Substantial experience within an acute hospital or hospice at Band 5 (or equivalent).
Excellent communication and interpersonal skills.
Own car and full UK driving licence.

Manage the Hospice at Home caseload on a daily basis ensuring appropriateness of referrals and delegating work to the Hospice at Home staff. Responsible for co-ordinating flexible care provision to enable patients to remain at home. Work in partnership with clients and deliver evidence based programmes of care. Act as a role model and supervise staff and provide mentorship for Registered Nurses and Nursing Assistants within the team, nursing students where appropriate, and colleagues.Develop and maintain effective communication networks with other health professionals to work flexibly and collaboratively across organisations and boundaries. Work in partnership with the clinical nurse specialist team- providing specialist advice and support under the supervision of the caseload holder Work within own area of clinical competency. Person Specification
Qualifications Essential
RGN Nurse Level 1 with current NMC registration. Healthcare related degree or working towards. Post registration education in palliative/end of life care or equivalent Previous palliative care/end of life care experience at Band 5.
Desirable
Teaching/ mentoring and assessing qualification Understanding of health and social care reform. Knowledge of hospice work. Ability to use Excel spread sheets. Experience of working in a community setting
