Job summary
Akerman Medical Practice is seeking an enthusiastic, patient-centered and committed Salaried GP to join our multidisciplinary team.
The post has been designed to provide a flexible portfolio-style GP role, combining continuity of care for care home residents with alternate Saturday clinical sessions.
The position may be particularly suitable for a GP who has existing weekday employment or portfolio commitments and wishes to supplement their current sessions with a regular, structured role incorporating weekend working and flexible care home responsibilities.The post-holder will be responsible for delivering safe, effective and evidence-based primary medical care while maintaining the highest professional and clinical standards.We are looking to develop a stable, long-term working relationship with the successful applicant.
Main duties of the job
The post consists of two care home sessions and alternate Saturday clinics. Working arrangements will be agreed with the successful candidate and Lead GP.
Care Home Session 1 Ward Round:A planned care home session including:
Scheduled ward rounds and resident reviews.Assessment and management of acute, chronic and complex conditions.Frailty and elderly care management.Medication and investigation reviews.Referrals and advance care planning where appropriate.Liaison with care home staff, relatives and multidisciplinary teams.Completion of associated administration and accurate EMIS Web records.Care Home Session 2 Flexible Support:Delivered flexibly across the week to manage issues arising between ward rounds, including clinical and medication queries, changes in residents' conditions, results, referrals, investigations, remote reviews and liaison with community or secondary care services.
Availability and workload will be agreed with the Lead GP to ensure safe and manageable working arrangements.
Alternate Saturday Clinics:Saturday clinics are an integral part of the role and will alternate between:
Remote: Telephone/remote consultations.Practice-based: A combination of telephone and face-to-face consultations at Akerman Medical Practice.The arrangement will continue according to the agreed rota.

Job description Job responsibilities
Provide high-quality general medical services to patients under the care of Akerman Medical Practice.
Take clinical responsibility for an agreed care home caseload.
Undertake a regular planned care home ward round.
Provide flexible clinical support for care home queries and concerns arising during the working week.
Undertake alternate Saturday clinical sessions as an integral part of the role.
Provide both remote and face-to-face consultations according to the agreed rota.
Work collaboratively with the wider multidisciplinary team.
Maintain high standards of clinical governance, prescribing, record keeping and patient safety.
Contribute to the continued development and improvement of clinical services within the practice.
Person Specification
Qualifications Essential
Essential Qualifications & Registration The successful applicant must: Be a qualified General Practitioner. Hold full GMC registration with a license to practice. Be included on the GMC GP Register. Be included on the appropriate NHS England Medical Performers List or meet the necessary eligibility requirements. Hold MRCGP or an equivalent recognised qualification. Maintain appropriate medical indemnity. Have an up-to-date NHS GP appraisal and revalidation status. Hold an Enhanced DBS certificate or be willing to undertake the appropriate check. Have the right to work in the United Kingdom. Essential Skills & Experience The successful applicant should demonstrate: Excellent clinical skills. Excellent communication and interpersonal skills. Ability to work effectively within a multidisciplinary team. Ability to work autonomously and make appropriate clinical decisions. Ability to manage a varied NHS primary care caseload. Ability to prioritise and manage workload effectively. High standards of clinical documentation. Safe and effective prescribing. Good IT skills. Experience with electronic primary care clinical systems. Understanding of NHS general practice and primary care. Understanding of clinical governance, safeguarding and patient safety. A flexible and collaborative approach to working.
Desirable
Desirable Previous experience of care home medicine. Experience managing frailty and complex multimorbidity. Interest in elderly medicine and/or dementia. Experience of advance care planning. Experience of multidisciplinary working across primary, community and social care. Experience using EMIS Web. Experience of quality improvement or clinical audit. Additional clinical interests that complement the needs of the practice. Interest in teaching, training or clinical leadership.
