Job summary
Fantastic opportunity for a Band 6 Home Therapies Specialist Nurse to join our small but close team who work together to enable the development of the Home Therapies service.
The successful applicant will provide highly specialist Renal care within the hospital and community setting to those patients with renal conditions who have complex needs, using highly specialist clinical knowledge and skills.
Main duties of the job
Provide holistic specialist nursing to patients with renal disease, assessing physical, psychological and social needs, implementing clinical management plans as agreed by renal multi-disciplinary team.
Provide one to one counselling to patients on life changing treatments, ensuring patients understand complex requirements of their chronic disease management and comply with treatment modality regimes.
Manage own day to day activity in relation to patient caseload and needs of service, ensuring balance of clinical, education and management achieved.

The role of Renal Nurse Specialist involves independent working between Arrowe Park, Clatterbridge Hospital, Countess of Chester Hospital and patients homes and therefore requires a broad spectrum of clinical and communication skills, including the use of multiple information technology systems and trust policies and procedures.
The role encompasses the clinical management of a range of patient cohorts including pre dialysis, peritoneal dialysis, in centre (self-care), home haemodialysis and renal vasculitis patients requiring intravenous chemotherapy. Each cohort is led and managed within the individual scope and competencies of the Renal Specialist Nurse.
In collaboration with Home Therapies Manager and within clinical competence, there will be overlap between each cohort, ensuring effective provision of all aspects of clinical care.
