Job summary
We are looking for a part time Practice Nurse to join our team at Swanscombe Health Centre and branch practices.
Based at Swanscombe Health Centre, the role of the Practice nurse will be to work within our nursing team, to offer support and treatment for our patients, focusing particularly on health improvement. You should have experience of working in a similar role, with relevant qualifications, NMC registration, and have excellent organisational skills.

We have a team of over 125 staff throughout 7 sites, including GPs, Paramedic Practitioners, Clinical Pharmacists, Physicians Associates, a Physiotherapist, Practice Nurses, Healthcare Assistants, Social Prescribers and the administrative support workers. Job description Job responsibilities
The main duties and responsibilities are as follows.
1. Meeting patients nursing care needs as identified, or where care has been transferred to them by a GP. This will usually be in surgery but occasional home visits will also be required.
2. Initiating and maintaining health screening and promotion clinics for identified areas of the practice population, recognising where modification of systems or referral is necessary.
3. Managing the nursing care of patients, employing personal recall systems suitable to the practice needs. This includes using the practice computer system, EMIS, for input of information for recall and screening purposes. Accurate and efficient input of data.
4. The following clinics:
Holiday Vaccination Clinic
Flu Vaccination Clinic
Cervical Smears
Family Planning
Baby immunisation Clinic
Wound Management
INR star
Assisting GP with Minor Operations
Amending or adding to services as needs are identified
5. Maintaining efficient liaison with all areas of the Primary Care Team and other agencies to assure continuity of care where necessary.
6. Routine treatment in general clinics, including:
Warfarin monitoring
Wound management
Removal of foreign bodies
ECGs
Ear syringe
And other general Treatment Room skills depending on the service need

Qualifications Essential
Must hold a nursing qualification and be registered with the NMC

Experience of working within a nursing team Experience of dealing with vulnerable patients Experience of a fast paced nursing environment
