Job summary
We are looking for an enthusiastic and committed Practice Nurse to join our friendly GP practice for 10 hours per week, working on Fridays.
This is an excellent opportunity for an experienced Practice Nurse looking for a regular, part-time role within a supportive and well-established practice team.
We recognise that Practice Nurses have different areas of experience and expertise, and we do not expect the successful candidate to have experience in every aspect of the role. We are keen to welcome someone who can bring their own skills and experience to complement our existing nursing provision.
You will have the opportunity to provide a varied range of nursing care, including preventative healthcare, long-term condition management, immunisations and health promotion, according to your training and competencies.

Main duties of the job
The successful candidate will provide a range of planned nursing care to our registered patient population, working alongside our existing Practice Nurse and wider clinical team.
The role will include routine nursing appointments, health promotion, immunisations, clinical observations, wound care and long-term condition reviews, depending on your experience and competencies.
You will support patients with the management of their health conditions, provide appropriate education and self-management advice, undertake relevant monitoring and investigations, and ensure patients receive appropriate follow-up or onward referral when required.
You will work autonomously within your professional scope of practice, whilst having the support of the wider practice team and access to GP colleagues when clinical advice or escalation is required.
You will also contribute to clinical governance, accurate record keeping, safeguarding, infection prevention and quality improvement within the practice.

JOB DESCRIPTION
To provide high-quality, evidence-based nursing care to patients registered with the Practice, working independently and in partnership with the GPs and wider multidisciplinary team.
The post holder will assess, plan, implement and evaluate nursing care and treatment within their professional competence, in accordance with current national guidance, Practice policies, agreed clinical protocols and the patient's informed consent.
The Practice Nurse will contribute to the delivery of preventative healthcare, health promotion, immunisation, screening and long-term condition management, whilst ensuring a high standard of patient safety, confidentiality and professional practice.
The exact range of duties undertaken will be dependent upon the post holder's qualifications, training, experience and demonstrated clinical competencies.
MAIN DUTIES AND RESPONSIBILITIES
The Practice Nurse will undertake appropriate clinical duties, which may include:
Assessing patients and providing appropriate nursing care and advice. Maintaining accurate, contemporaneous and comprehensive patient records, both electronic and written where applicable. Undertaking clinical observations including blood pressure, pulse, temperature, respiratory rate, oxygen saturation, height, weight and BMI as clinically appropriate. Venepuncture and the appropriate collection, handling and processing of specimens. Performing ECGs where trained and competent. Undertaking spirometry where appropriately trained and competent. Providing wound care and dressing procedures. Providing health promotion and lifestyle advice. Providing smoking cessation advice and signposting to appropriate services. Recognising and appropriately managing clinical emergencies, including anaphylaxis, in accordance with Practice procedures and current guidance. Escalating clinical concerns promptly to an appropriate clinician. Providing patients with appropriate education and advice to support self-management. Working within the limits of professional competence and escalating where additional clinical expertise is required.
Immunisation and Vaccination
The Practice Nurse may be responsible for:
Administration of routine adult and childhood immunisations in accordance with current national guidance and Practice protocols. Administration of travel vaccinations where appropriately trained and competent. Supporting seasonal influenza and other vaccination programmes. Assessing vaccination history and identifying patients requiring appropriate immunisation. Providing patients with information and advice regarding vaccination. Recognising and appropriately managing adverse reactions and vaccination-related emergencies. Ensuring accurate recording of vaccinations and appropriate clinical coding.
Long-Term Condition Management
The Practice Nurse will contribute to the monitoring and management of patients with long-term conditions, according to training and competence.
This may include:
Asthma. COPD. Diabetes. Hypertension. Ischaemic heart disease and other cardiovascular disease. Atrial fibrillation. Chronic kidney disease. Other long-term conditions managed within primary care. Responsibilities may include:
Undertaking structured clinical reviews. Monitoring relevant clinical observations and investigations. Providing lifestyle and self-management advice. Assessing medication adherence and identifying concerns for appropriate clinical review. Reviewing inhaler technique where appropriate. Supporting personalised care planning. Identifying patients requiring further assessment or intervention. Arranging appropriate follow-up and recall. Referring or escalating to GPs or other healthcare professionals where clinically indicated. Ensuring appropriate clinical coding and record keeping. The post holder will support the Practice in delivering relevant quality improvement and preventative healthcare programmes.
Screening and Health Promotion
The Practice Nurse will contribute to preventative healthcare and health promotion, including where appropriately trained:
NHS health checks and cardiovascular risk assessment. Cervical screening. Sexual health and contraception advice. Travel health advice. Lifestyle advice including smoking cessation, diet, exercise and weight management. Health promotion and patient education. Screening and early identification of health concerns. Signposting and referral to appropriate services. Specialist procedures will only be undertaken where the post holder has appropriate training, experience and competency.
Sexual and Reproductive Health
Where appropriately trained and competent, the post holder may provide:
Contraceptive advice. Sexual health advice. Contraception-related monitoring and follow-up. Cervical screening. Appropriate referral to specialist sexual health services.
Patient Education and Self-Management
The Practice Nurse will:
Provide patients with appropriate information and education regarding their health and wellbeing. Encourage patients to take an active role in managing long-term conditions. Support patients in developing appropriate self-management strategies. Provide written and verbal information appropriate to the patient's needs. Signpost patients to relevant support services, community services and educational resources. Consider health literacy, communication needs and individual circumstances when providing advice.
Care Planning and Follow-Up
The post holder will:
Arrange appropriate follow-up appointments. Participate in patient recall systems. Ensure appropriate monitoring and review is undertaken. Contribute to personalised care and care planning. Support proactive identification of patients requiring review. Contribute to appropriate care planning for patients at risk of deterioration or hospital admission.
Clinical Governance and Quality Improvement
Participate in clinical audit and quality improvement activities. Contribute to the development, review and updating of Practice clinical protocols and guidelines. Work in accordance with current evidence-based guidance and Practice policies. Participate in significant event reviews and learning where appropriate. Identify and report clinical risks, incidents and near misses. Contribute to maintaining high standards of patient safety and quality of care. Ensure clinical equipment is used safely and appropriately. Maintain appropriate infection prevention and control standards.
Stock and Equipment
The post holder will:
Ensure clinical equipment is maintained, clean and fit for purpose. Report faulty or defective equipment promptly. Assist with maintaining appropriate levels of clinical stock. Ensure vaccines, specimens and other clinical materials are stored and handled appropriately. Assist in monitoring stock levels and reporting requirements to the appropriate member of staff. Ensure clinical areas are maintained in a safe and appropriate condition.
Safeguarding
The Practice Nurse will:
Recognise and appropriately respond to safeguarding concerns involving children, young people and adults at risk. Follow Practice safeguarding policies and procedures. Escalate concerns appropriately to the Practice safeguarding lead or relevant external service. Maintain accurate and appropriate safeguarding documentation. Undertake safeguarding training appropriate to the role.
Confidentiality and Information Governance
The Practice Nurse must maintain strict confidentiality in relation to patients, staff and Practice business.
Comply with UK GDPR, the Data Protection Act 2018 and Practice information governance policies. Ensure patient information is accessed, recorded and shared appropriately. Always maintain confidentiality. Ensure clinical records are accurate, relevant and appropriately maintained. Follow Practice procedures relating to information security and data protection.

Experience Essential
1. Working as a Registered Nurse. 2. Assessing patients and providing appropriate nursing care. 3. Working both independently and as part of a multidisciplinary team. 4. Maintaining accurate clinical records.
