We are seeking an experienced and motivated Physiotherapist to join our Unplanned Care Service in Hertfordshire on a temporary bank basis, working 24 hours per week, Monday to Friday.
This is an exciting opportunity for a Physiotherapist with experience in community settings, discharge planning, equipment prescription and provision, and lone working. The successful candidate will primarily work across the Discharge to Assess (D2A) and Rapid Response pathways, with the flexibility to support other pathways when required.
You will work as part of a multidisciplinary team alongside physiotherapists, occupational therapists, nurses and paramedics, delivering high-quality, patient-centred care to support timely discharge and prevent unnecessary hospital admissions.
Main duties of the job
Deliver a wide range of specialist physiotherapy assessment and treatment as part of the Unplanned Care Service. Work with patients with a range of complex needs, supporting timely discharge from acute care and preventing hospital admission. Provide a comprehensive physiotherapy service to residents of West Hertfordshire referred into the service. Work primarily across the Discharge to Assess and Rapid Response pathways, with the ability to work across other pathways according to service requirements. Assess and manage an autonomous caseload, developing and implementing appropriate treatment and rehabilitation plans. Support patients to maximise their independence and achieve their rehabilitation goals within their own home or community environment. Contribute to effective discharge planning, working collaboratively with patients, families, carers and the wider multidisciplinary team. Undertake equipment assessment, prescription and provision where clinically appropriate. Work confidently as a lone practitioner within community settings, recognising and managing risk appropriately. Maintain accurate and timely clinical documentation in accordance with professional standards and organisational requirements. Work within professional standards, clinical guidelines and best practice to ensure safe and effective patient care. Contribute actively to the wider clinical team and multidisciplinary decision-making.

Education/Qualification Essential
Diploma/degree in Physiotherapy or Occupational therapy Current HCPC registration Evidence of up-to-date CPD portfolio Involvement in Appraisal Scheme Attendance at specialist short courses or equivalent CPD activities within the specialist areas of Adult Rehabilitation, Neurology, Orthopaedics, Musculoskeletal Outpatients, Older People Rehabilitation Member of Professional Body

Experience Essential
Experience post registration as a Band 5 Physiotherapist or Occupational therapist Experience working at a Junior level in a wide range of specialised clinical areas including outpatients, neurology, rehabilitation, orthopedics and respiratory.

Skills & Knowledge Essential

Key Attributes Essential
