About this role

*Job Summary*

Join our team supporting our GP in his family practice with day to day patient care, PAP smears, immunizations, vaccines, etc.

*Responsibilities*

* Conduct thorough patient assessments, including history-taking and physical examinations, with particular attention to pediatric and senior populations. * Develop and implement individualized treatment plans, including medication management and health education. * Administer medications, perform procedures such as phlebotomy, and oversee pain management strategies effectively. * Coordinate care with specialists and other healthcare providers through case management to ensure seamless patient experiences. * Manage chronic conditions such as dementia, dementia care, and acute illnesses by applying evidence-based practices. * Utilize electronic health record systems like Epic or Cerner for documentation, order entry, and patient tracking. * Provide patient care in various settings including clinics, nursing homes, assisted living facilities, home care environments, and hospitals. * Support laboratory testing procedures and interpret results to inform clinical decisions. * Engage in patient education on topics like medication adherence, lifestyle modifications, and preventive health measures. * Maintain compliance with medical terminology standards and ensure accurate communication across multidisciplinary teams.

*Skills*

* Proven experience working with children (pediatrics) and senior care populations. * Extensive nursing home experience combined with hospital experience for comprehensive acute care delivery. * Strong background in pain management, dementia care, and home care services. * Skilled in case management practices to coordinate complex patient needs effectively. * Proficiency in medical terminology, medication administration, phlebotomy, laboratory procedures, and laboratory experience. * Familiarity with electronic health records systems such as Epic or Cerner for efficient documentation. * Excellent patient care skills with a compassionate approach tailored to diverse age groups and conditions. * Knowledge of assisted living environments and familiarity with medical office workflows.

Pay: $55.00-$65.00 per hour
