• Ability to navigate government, health, and nonprofit websites and explain available resources clearly
• Experience in libraries, social services, public health, healthcare, community outreach, education, or a related field is preferred
• Familiarity with Medicare, Medicaid, SNAP, and New Jersey and Passaic County health and human-service resources is desirable
Key Responsibilities:
• Develop and maintain circulating Health Information Totes containing books, handouts, brochures, and other educational materials on health, wellness, caregiving, aging, nutrition, mental wellness, preventive health, and related topics.
• Ensure health information and resources are current, reliable, and appropriate for public use.
• Coordinate monthly visits by the Library's social worker, including scheduling confidential patron appointments and promoting the service.
• Assist patrons in locating information about Medicare, Medicaid, SNAP, and other health and human-service programs using official websites, informational materials, and agency resources.
• Maintain information about local, county, state, and federal health and social-service resources and direct patrons to appropriate agencies and professionals for additional assistance.
• Assist the Library Director in identifying community health needs and developing appropriate programs, workshops, resource fairs, and educational opportunities.
• Serve as the Library liaison with Passaic County Human Services and related county health and human-service agencies.
• Coordinate visits by county representatives to provide information, outreach, educational programs, enrollment assistance, and other appropriate services at the Library.
• Develop and maintain a partnership with the Berkeley College Medical Department to coordinate educational presentations, health-related programs, and services for Library patrons.
• Develop relationships with healthcare providers, nonprofit organizations, government agencies, and other community partners that can provide beneficial programs, information, and resources.
• Coordinate community health and wellness presentations, workshops, screenings, resource fairs, and outreach opportunities.
• Promote available health and social-service resources through Library programs, displays, handouts, and other appropriate outreach.
• Confidentiality is an essential requirement of this position. The Health & Human Services Navigator may assist patrons with sensitive personal matters and must protect the privacy and confidentiality of all patron interactions.
• The employee will:
• Maintain confidentiality regarding patron requests, appointments, circumstances, and personal information.
• Collect or retain only the minimum information necessary to coordinate services or appointments.
• Follow all Library policies regarding patron privacy, confidentiality, records, and computer use.
• Refer confidentiality concerns, emergencies, or situations requiring professional intervention to the Library Director or appropriate professional.
