Job summary
SLC Medical Group are seeking a Nurse Practitioner to join our established clinical teams working across our 3 sites in Grimsby.
Responsible for ensuring the provision of effective, efficient and appropriate high quality patient care including diagnosing and treating patients presenting with minor illness, consulting and advising people in respect of their continuing medical and nursing needs and to provide care and support to patients in a General Practice setting.
The Nurse Practitioner will need to prioritise and triage the needs of patients accordingly making any necessary referrals for investigations in the appropriate manner. Patients with problems needing referral to secondary care should be discussed with the registered GP before making such referral.
Main duties of the job
The post-holder will be a registered Nurse, and will hold a diploma in Minor Illness and have experience in diagnosing and treating patients from the very young to the elderly.
Experience in triaging patients in a General Practice setting would be advantageous.

The nurse practitioner will support the lead NP and GP Partners with appropriate clinical administrative duties including QOF management, medication reviews, medical reports and referral management within the remit of the role.
Job Responsibilities:
Diagnosing and treating patients presenting with minor illness
Treat patients once triaged making any necessary referrals to other members of the primary health care team. To support the on call doctor assessing/planning/implementing and evaluating patient care. To support the practice triage system by performing nurse triage where appropriate. Ensure clinical practice is safe and effective and remains within boundaries of competence and to acknowledge limitations. Advise patients on general health care and minor ailments with referral to other members of the primary and secondary health care team as necessary. Support GPs with medication reviews. Participate in the administrative and professional responsibilities of the practice team. Ensure accurate and legible notes of all consultations and treatments are recorded in the patients notes. Ensure the clinical computer system is kept up to date with accurate details recorded and amended as per practice protocol. Ensure appropriate items of service claims are made accurately, reporting any problems to the practice administrator. Ensure accurate completion of all necessary documentation associated with patient health care and registration with the practice. Ensure collection and maintenance of statistical information required for regular and ad hoc reports and audit. Attend and participate in practice meetings as required. Restocking and maintenance of clinical areas and consulting rooms. Responsible for checking own pathologies and ensuring appropriate action takes place. Support GPs with medical reports/insurance reports, QOF management, referral management within own remit. Supplies and Equipment:
Maintenance of equipment and stock relating to patient care. Training and Personal Development:
Training requirements will be monitored by monthly supervision and yearly Personal Appraisal and Development Review (PADR) and will be in accordance with practice requirements. Personal development will be encouraged and supported by the practice. It is the individuals responsibility to remain up to date with recent developments. Participate in the education and training of junior staff and medical students of all disciplines and the induction of members of the practice staff where appropriate. Assist in the development and maintenance of an in house nurse-training programme reporting to the Nurse Practitioner and Practice Manager. Maintain continued education by attendance at courses and study days as deemed useful or necessary for professional development ensuring PREP requirements are met. Attendance at the North East Lincolnshire Primary Care Programme where appropriate. If it is necessary to expand the role to include additional responsibilities, full training will be given. Develop and maintain a Personal Learning Plan. Liaison:
As well as the nursing team there is a need to work closely with all surgery staff to ensure the smooth running of the practice, reporting any problems encountered to the relevant person and ensuring every one is aware of the different roles within the nursing team. There is also the need to establish and maintain good liaison with other surgeries and agencies including secondary care.

RCN Qualified A recognised Minor Illness course or equivalent
