About this role

Job summary

Hatters Health PCN are offering an exciting opportunity for a clinical coder to join a newly forming centralised coding team. Working to support our 5 friendly, supportive member practices. The initial term is 1 year fixed contract with a view to a permanent role.

The successful candidate will have strong attention to detail with a an organised and methodical approach to work.

Main duties of the job

The Clinical Coder will be responsible for:

Identifying relevant clinical information documented in patient correspondence, coding appropriately and completing any detailed action Identifying and updating relevant clinical information from incoming correspondence Accurately coding diagnoses, procedures and relevant information into patient records Highlighting all timely actions, changes to patient care and management to the relevant clinician. Including level of urgency as appropriate Liaising with clinicians and administrative staff when clarification or further actions required Filing and work flowing documents where appropriate Correcting historic/ incorrect coding Data cleanse the problem and summary pages in the clinical record Maintaining strict patient confidentiality at all times

Roles and Responsibilities

This role is an important part of ensuring patient records are accurate, up to date and coded correctly to support safe clinical care and practice reporting.

The Clinical Coder will be responsible for:

Identifying relevant clinical information documented in patient correspondence, coding appropriately and completing any detailed action

Identifying and updating relevant clinical information from incoming correspondence Accurately coding diagnoses, procedures and relevant information into patient records Highlighting all timely actions, changes to patient care and management to the relevant clinician. Strong attention to detail and accuracy Good IT skills and ability to use clinical systems Ability to understand and interpret medical terminology Organised and methodical approach to work Understanding of confidentiality and data protection

Desirable:

Previous experience in a GP surgery or NHS setting Knowledge of clinical coding systems (e.g. SNOMED CT / Read Codes) Experience using SystmOne or similar systems Clinical coding or medical terminology qualification

Experience of handling sensitive information
