About this role

Job summary

The post holder will have a service wide remit to support services to deliver excellent palliative and end of life care throughout In-Patient Unit (IPU) Community Services (CNS Team) and the Day services/ Wellbeing services. This post holder will also be expected to deliver teaching and supervision to the Advanced Clinical Practitioners (ACPs) and Clinical Nurse Specialist (CNS)

This post holder will report directly into the Director of Clinical Services, and access supervision via the resident consultant and the GMC. The post holder will work with the resident consultant to create networks with other hospice and palliative care colleagues to strengthen the alliances and knowledge base for the benefit of the patients.

Main duties of the job

KEY TASKS

Maintenance of high quality, evidence-based, Palliative Care to patients and their families with care, compassion and dignity

Responsibility for the day-to-day clinical management of patients including hospice in-patients and community outpatients

To participate in the out-of-hours on call rota Bassetlaw Hospice as and when required.

Participate in clinical governance and quality improvement alongside the resident consultant.

Work to achieve personal and Hospice objectives

Ensure active participation in continuing professional development and engage in appraisal and revalidation

To participate in the delivery of education and training both within the hospice and externally

Contribute to educational and clinical supervision of trainees (E.G ACP/ other medical professionals)

To support research activities in Specialist Palliative Care as appropriate.

About us

Bassetlaw Hospice is a newly commissioned provider of palliative and end of life services. Whilst Bassetlaw Hospice has been open for over 30 years, the service has been delivered through Nottinghamshire Healthcare Trust for the past few years, but it is now an independent charity run service with partial funding via the ICB

Bassetlaw Hospice, Retford, North Nottinghamshire has 6 inpatient beds supported clinically by established Advance Clinical Practitioner roles.It also provides clinical nurse specialist support to the community as well as day therapy and outpatient clinics. There are excellent links with local secondary care, primary care and CCG colleagues as well as aspects of palliative care delivered via the Nottinghamshire Healthcare Trust community Services and the end of life alliance.

Job description Job responsibilities

As a GP working within a Hospice and palliative care you will provide the following:

Deliver day-to-day medical care to hospice in-patients and patients accessing other Specialist Palliative Care services.

Provide clinical leadership, education and development to the Advanced Clinical Practitioners alongside the resident consultant.

Provide leadership and participate as a member at the weekly in-patient multidisciplinary team meeting.

Out-of-hours responsibilities as required/ involvement in a wider on call roster.

Cover for colleagues annual leave and other authorised absences.

Any responsibility which relates to a special interest.

Educational and clinical supervision of trainee doctors, advanced clinical practitioners.

Training and supervision of Nurse Independent Prescribers.

Participate in clinical audit, quality improvement projects and clinical governance processes.

Support research activity.

Comply with Continuing Professional Development (CPD) requirements as stipulated by the Royal Colleges.

To keep a portfolio of evidence, to have an annual appraisal and write a personal development plan (PDP) in line with GMC revalidation policies and procedures.

Support and assist in the achievement of Hospice strategy and objectives.Proposed Job Plan

The initial job plan will be agreed flexibly with the successful candidate and will be subject to negotiation and review. Three months after commencement, the job plan will be reviewed and agreed between the appointee and the Director of Clinical Services. This will be a prospective agreement that sets out the duties, responsibilities and objectives for the coming year, and will be reviewed annually. It should cover all aspects of a GPs professional practice including clinical work, teaching, research, education and managerial responsibilities. In addition, it should include personal objectives, including details of their link to wider service objectives, and details of the support required by the consultant to fulfil the job plan and the objectives.A shared office will be provided with some secretarial support and informational technology facilities. Time off in Lieu will be available for any additional weekend or out of hours work, as agreed by line manager.

On call availability

Out of hours activity is currently delivered by Derbyshire Health United (DHU) practitioners who then access on call consultants via South Yorkshire or Nottinghamshire. This will be reviewed and developed in line with service need. It is possible that an on call requirement may become necessary in the future.

Supporting Programmed Activities

The role will require oversight of clinical education for Palliative and End of Life Care across Bassetlaw Hospice and to colleagues in partner organisations, to ensure services are educated and fit for purpose. The appointee is expected to participate in teaching in collaboration with the education lead.The post holder will be encouraged to contribute to research activities at the hospice and contribute to audit and quality improvement projects.The appointee is expected to take part in continuing professional medical education; study leave is provided and funding is available on application.

Person Specification

Qualifications Essential

Full registration with the UK General Medical Council with a licence to practise MRCP

Desirable

Higher qualifications relevant to Palliative Medicine eg M.D., MSc, Ph.D Training or qualification in medical education

Experience Essential

Ability to offer expert clinical opinion on a range of Specialist Palliative Care problems Ability to take full and independent responsibility for clinical care of patients Ability to teach clinical and non-clinical skills to undergraduates and postgraduates Supervision of doctors in training and non-training grade posts. Supervision of trainee ACPs and CNSs

Desirable

Experience of working within the Bassetlaw area to have an understanding of wider services and the impact on patient care and journey. Training or qualification in medical education Evidence of engagement with and skills in research Evidence of attendance on an Advanced Communication Skills Course
