Position Title: Intake Specialist
Job Summary: The Intake Specialist is responsible for managing the intake process for home care clients from initial referral through the start of services. This role involves receiving and processing referrals, collecting and maintaining accurate client information, coordinating assessments, verifying eligibility and insurance coverage, and ensuring timely communication and coordination among clients, caregivers, healthcare providers, and internal teams.
Work Schedule: On-Site | Monday - Friday, 9:00 AM - 5:00 PM
Job Responsibilities:
• Receive and process new client referrals and intake requests.
• Collect and verify client demographics, medical information, insurance details, and required documentation.
• Coordinate initial client assessments and start-of-care services.
• Communicate effectively with clients, families, healthcare providers, caregivers, and internal care teams.
• Maintain accurate and up-to-date client records and documentation in the system.
• Coordinate caregiver assignments and service schedules as needed.
• Follow up on referrals and ensure all intake requirements are completed accurately and timely.
• Ensure compliance with company policies, HIPAA regulations, and applicable home care laws and regulations.
• Provide excellent customer service and respond promptly and professionally to client and family inquiries.
Job Qualifications:
• College degree required.
• At least 1 year of previous experience in home care, healthcare, or intake is required.
• Bilingual in English and Russian required; proficiency in additional languages is a plus.
• Health Insurance, Medicare, or Medicaid agent experience is a plus.
• Experience with Salesforce and HHAeXchange is preferred.
• Strong communication and interpersonal skills, with the ability to interact professionally with clients, families, healthcare providers, and internal teams.
• Excellent organizational skills and strong attention to detail.
• Ability to multitask, prioritize responsibilities, and work effectively in a fast-paced environment.
• Proficiency with computers, electronic medical records (EMR), and Microsoft Office.
• Ability to handle confidential and sensitive information with professionalism and discretion.
