About this role

Position Summary

Soma Medical Center is seeking an organized, dependable, and patient-focused Referral Coordinator to manage and coordinate patient referrals to specialists, diagnostic facilities, and other healthcare providers. The Referral Coordinator plays an important role in ensuring patients receive timely access to appropriate care while maintaining accurate documentation and communication between patients, providers, insurance companies, and outside facilities.

Key Responsibilities

• Process and coordinate incoming and outgoing patient referrals accurately and in a timely manner. • Obtain necessary insurance authorizations and prior approvals for specialist visits, procedures, diagnostic testing, and other services. • Verify patient insurance information, eligibility, and referral requirements. • Schedule or assist patients with scheduling appointments with specialists and outside facilities. • Communicate referral information, appointment details, and instructions clearly to patients. • Send relevant medical records, physician orders, test results, and supporting documentation to specialists and facilities. • Follow up on pending referrals and authorizations to ensure continuity of patient care. • Track referral status and document all actions and communications accurately in the patient's electronic medical record (EMR). • Communicate with physicians, medical assistants, front desk staff, insurance companies, specialists, and other healthcare organizations regarding referral needs. • Identify referral or authorization issues and work to resolve them promptly. • Obtain specialist consultation notes and other documentation when necessary to ensure appropriate follow-up care. • Maintain patient confidentiality and comply with HIPAA, OSHA, and all applicable Soma Medical Center policies and procedures. • Provide professional, courteous, and compassionate customer service to patients. • Perform additional administrative or patient coordination duties as assigned by management.

Qualifications

• High school diploma or equivalent required. • Previous experience in a medical office, referral, authorization, or patient coordination role preferred. • Knowledge of medical terminology, insurance plans, referrals, and prior authorization processes preferred. • Experience working with electronic medical records (EMR/EHR) systems preferred. • Strong organizational and time-management skills with the ability to manage multiple referrals and deadlines. • Excellent communication and customer service skills. • Strong attention to detail and ability to maintain accurate patient records. • Ability to work effectively both independently and as part of a healthcare team. • Bilingual in English and Spanish required. • Basic computer proficiency and ability to learn medical office software and systems.
