Job summary
Bridge End Surgery has an exciting opportunity for an enthusiastic, experience Practice Nurse to join our friendly and committed nursing team. The position is for 24-28 hours per week.
You will be joining a great team working alongside 2 Nurse Practitioners, 3 Practice Nurses, a Healthcare Assistant and GPA
The Practice Nurse will work delivering patient-centred long term conditions management to our patients. You will work both independently and as part of a multi-disciplinary team, using expert clinical judgement and decision making to ensure safe and effective care.

The Practice Nurse wilk work autonomously and as part of the wider team to deliver high quality care, including
Management of long term conditions, including diabetes, asthma, COPD and Ischaemic Heart DiseaseProviding follow up and continuity of care for patientsDelivering health promotion, preventative care, and self - management supportSupporting the practice in achieving QOF targets and enhanced servicesContributing to service development and quality improvement initiatives
About us
The practice comprises of 2 GP Partners and 6 salaried GP's, 2 ANP's, 3 Practice Nurses, 1 HCA and 1 GPA. We also have an established highly skilled reception and admin team.
As a practice, we support the nursing team throughout their professional development as well as also being a training practice for Doctors in Training and Medical Students. The practice holds a strong belief in the importance of continuous professional development as well as personal wellbeing.
We work closely with our PCN colleagues including Pharmacist's, Pharmacy Technicians, First Contact Physio, Mental Health First Contact, CAMHS Mental Health Nurse.

Person Specification
Qualifications Essential
Registered Nurse (NMC) Significant primary care or relevant experience Management and education of patients with chronic disease/long term conditions Evidence of ongoing CPD and revalidation Certified in immunisations and vaccinations Cervical smear taking
Desirable
Experience of working with EMIS clinical system Knowledge of QOF, NICE guidance and national screening programmes Excellent interpersonal and teamworking skills Excellent time management skills/ability to prioritise and manage own time
