About this role

Job summary

We are seeking a dedicated, forward-thinking General Practitioner to join our Primary Care Network (PCN) in a unique five-session weekly role that balances frontline patient care with proactive care and health inequality work. The position is an ideal opportunity for a GP looking to broaden their clinical portfolio while making a lasting impact on population health across our diverse communities.

The clinical component consists of four sessions per week dedicated to delivering high-quality, patient-centered care. You will primarily be based across two welcoming sitesGleadless Medical Centre and Sharrow Lane Medical Centrewith potential for flexible deployment across our wider PCN to meet operational needs if required. Your schedule will be managed so you are never working across more than two sites simultaneously. There will be access to senior GPs for advice and support.

Your fifth session is dedicated to clinical work within our PCN-based services. In this capacity, you will take the lead on innovative projects with a core focus on proactive care and tackling health inequalities. A key part of this time involves driving the continued development of our proactive care model, which is designed specifically to support frequent service users and those who experience structural barriers to healthcare.

Main duties of the job

This role will include:

5 sessions / week across 3 days

Tuesday Two sessions

Wednesday one session

Friday Two sessions

-Good supervision and mentorship with senior PCN GPs and managers

-Working as part of our PCN MDTs

-Opportunity to develop leadership skills (work alongside PCN Clinical Director and PCN Manager)

-Time for personal development, and non- patient facing learning

-Mix of core General Practice clinics in practices and more innovative services (as above)

Our PCN team comprises of clinical (includes mental health workers, pharmacists and pharmacy assistants, first contact physio, Advanced Care Practitioner, GP assistants and non-clinical roles (social prescribers and health coaches).

In accordance with the agreed PCN timetable, the post-holder will undertake a variety of duties, including:

Conducting surgery and telephone consultations, addressing patient queries, and visiting patients at home.

Checking, signing, and authorising prescriptions, while managing queries, paperwork, and correspondence in a timely manner.

Making professional, autonomous decisions regarding presenting problems, whether self-referred or referred by other health care workers.

Assessing the healthcare needs of patients with undifferentiated and undiagnosed issues.

Screening patients for disease risk factors and early signs of illness.

Developing care plans in consultation with patients, in line with current disease management protocols.

Providing counselling and health education.

Admitting or discharging patients from the caseload and referring them to other care providers as appropriate.

Recording clear and contemporaneous consultation notes to agreed standards.

Collecting data for audit purposes.

Attending MDT meetings

Participation in education/teaching of wider MDT teams including medical and non medical teams

Compiling and issuing computer-generated acute and repeat prescriptions, avoiding hand-written prescriptions whenever possible.

Prescribing in accordance with the Practice prescribing formulary or generically, as clinically appropriate.

Undertaking emergency patient visits during surgery hours, if clinically indicated.

Participating in on-call sessions and home visiting sessions as allocated in the practice rota.

Adhering to all relevant Practice policies and guidelines, including those related to prescribing, confidentiality, data protection, and health and safety.

Committing to lifelong learning and audit to ensure evidence-based best practices.

Contributing to evaluation, audit, and clinical standard setting within the organisation.

Assisting in the development of computer-based patient records.

Leading proactive and personalised care projects for the PCN and being an advocate for these services to patients and staff across the network

Person Specification

Qualifications Essential

Qualified General Practitioner Registered General Practitioner (GMC) Current UK/EU Driving License Registered on local Performer's List Willingness to complete an Enhanced DBS Check Up-to-date vaccinations Not previously been substantively employed at a GP Practice in Heeley Plus Network (over the last 12 months) Awareness of the primary care contract

Experience Essential

Experience of working in a primary care environment Experience of continued professional development Experience of QOF and clinical audit Experience of leading projects and delivery of person centred outcomes

Desirable

Experience of using Systmone/EMIS clinical systems
