About this role

Job summary

This is a fantastic opportunity to apply for an innovative role. We are seeking a Health and Wellbeing Coach to continue to develop the partnership between Platt Hall and Better Health MCR. The post holder will build on an established programme of health work at Platt Hall and will be embedded equally, working in both organisations. They will work at both sites to reduce health inequalities using Platt Hall's programmes, collections and building as a key resource. The post holder will take a lead in supporting people to self-identify issues and encourage proactive prevention of new and existing illnesses, with a focus on using evidence-based strategies.

Main duties of the job

The Health and Wellbeing Coach will take an approach that considers the whole person in addressing existing issues, working alongside people to coach and motivate them through multiple sessions, supporting them to identify their needs, set goals and help them to implement their personal health and care plan. They will develop creative arts-based wellbeing groups at Platt Hall in partnership with patients. They will take an approach that is non-judgemental, that supports personal choice and positive risk-taking, that addresses potential consequences, and ensures patients understand the accountability of their own decisions. The post holder will develop and maintain positive relationships with key stakeholders, providers and local communities.

You will support patients to implement their personalised health and care plan using the Platt Hall building, collections and programmes as a resource.

This is a fixed term post for two years which may be increased with additional funding.

We are a friendly, forward-thinking, not-for-profit social enterprise providing primary care medical services in Manchester. We operate three GP practices: The Robert Darbishire Practice in Rusholme Health Centre (which is also head office) and which has a branch at The University of Manchester, Brooklands Medical Practice in Wythenshawe and The Whitswood Practice in Alexandra Park Health Centre. In total we have over 44,000 patients.

Our patients are very diverse and include established communities as well as new communities from all over the world. We have good links with the University and a sizeable student population.

Our staff team of about 140 people has a huge range of fantastic skills and experience. Our shared (ARRS) staff are fully integrated into our services, providing a seamless service for patients.

Platt Hall is undergoing a long-term redevelopment programme, working with local partners and residents to create a new kind of museum and creative space focused on collections, creativity and culture as a tool for social change. Health equality is a key strand of this work. The Platt Hall team currently work in partnership with Better Health MCR to develop and recruit for a range of wellbeing programmes in the Hall, garden and park.

The post is full-time (37.5 hours per week) and will be mostly based at The Robert Darbishire Practice in Rusholme Health Centre and Platt Hall in Fallowfield, although you will need to be able to travel to our other sites if needed. The usual working hours are 08:30-17:00, Monday to Friday.

Salary will be in the range £32,073 to £39,043 depending on experience (pro rata to full time of 37.5 hours per week). We offer membership of the NHS Pension Scheme and 5.4-6.6 weeks' holiday (depending on NHS service) plus bank holidays (pro rata). Person Specification

Qualifications Desirable

Coaching/counselling qualification/experience or other relevant qualification/experience involving reflective listening skills relevant training and experience in non-clinical Supported Self-Management or Health Coaching. Level 3 Diploma in Exercise Referral or equivalent qualification. Ability to speak a second language that resonates with the demographics of the local neighbourhood.

Personal Skills and Abilities Essential

The ability to give good and regular attendance. Highly motivated, creative, enthusiastic and flexible in approach to work. A friendly and approachable manner with good people skills showing care and empathy. The ability to work independently and as a member of two busy multidisciplinary teams. Excellent communication skills the ability to communicate clearly and effectively with a wide range of individuals both verbally and in writing. Understanding of the importance and process of helping people with long-term conditions to develop their knowledge, skills and confidence in managing their health and the range of models and tools available. Understanding of health coaching in group settings. The ability to communicate and interact with the public in a discreet and sensitive manner, recognising their needs for alternative methods and styles of communication. The ability to communicate complex and sensitive information in an understandable form to a variety of audiences (e.g. patients). Good organisational skills. A strong commitment to providing a high standard of care that is responsive and flexible in its approach to meeting the needs of a deprived and ethnically diverse population. A strong commitment to reducing health inequalities and to supporting people to reach their potential. Willingness and aptitude to be involved in teaching. A commitment to Better Health MCR and Manchester City Council core values. A commitment to professional and personal development, with evidence of recent study and learning. An ability to understand, assess and manage risk. Produce timely and informative reports. Ability to work with minimal supervision and act decisively and ask for help when needed.

Experience Essential

Previous experience delivering coaching support or similar experience of community facilitation in a community development context, health and social care, learning support or public health / health improvement. Experience of working in health and social care care/community development setting or similar. An active interest in creative practice and awareness of the use of museum and gallery collections for wellbeing. Demonstratable understanding of Creative Health and use of art for wellbeing. Experience of partnership / collaborative working and of building relationships across a variety of organisations. Understanding of areas of high deprivation and the challenges this presents. Commitment to undertake a health coach qualification. Commitment to professional and personal development. Able to work from an asset-based approach, building on current community and personal assets. Good ICT and keyboard skills.

Desirable

Experience of using coaching approaches/frameworks and models or other helping strategies e.g., motivational Interviewing. Experience in supporting people with long-term health conditions to make lifestyle changes.
