An exciting opportunity has arisen for an experienced ANP to join our Home Visiting Team, working in and around Boston. You will be an enthusiastic and motivated ANP who wishes to influence patient care in this bespoke area of primary care.
Main duties of the job
Your main duties will be to assist daily with home visits including visit triage, seeing on the day patients for acute and minor illness, for housebound patients and care home residents, working on behalf of Boston Primary Care Network (PCN) practices.

An exciting opportunity has arisen for an experienced ANP to join our Home Visiting Wellbeing Team, working in and around Boston. You will be an enthusiastic and motivated ANP who wishes to influence patient care in this bespoke area of primary care.
Your main duties will be to assist daily with home visits including visit triage, seeing on the day patients for acute and minor illness, for housebound and care home patients, working on behalf of Boston Primary Care Network (PCN) practices.
You will be employed and supported by Boston PCN Ltd and will work with a diverse, multi-skilled, experienced professional team of clinical and non-clinical, friendly and committed staff.
Boston PCN covers six practices across Boston, Kirton and Swineshead areas.
Person Specification
Qualifications Essential
PLEASE SEE JD FOR FULL DETAILS Registered with NMC as RGN, with NMP qualification and evidence of masters level education for advanced clinician practice or working towards, including advanced examination skills Experience of working within a demanding primary care clinical setting, preferably GP practice. Independent prescriber Full and clean UK Driving Licence and access to transport.
Desirable
Experience of working in a community health / home visiting care setting linked to GP Practice. Experience of working in a multi-discipline health care team. Experience working within General Practice.
