Job summary
Murton Medical Group is looking to expand our team and is seeking an enthusiastic, patient-focused Clinical Pharmacist to join our busy, friendly and supportive GP practice in Murton, County Durham.
This is a part-time position of 16 hours per week, working across two days Monday and Tuesday. This is an exciting opportunity to become an integral part of our multidisciplinary practice team and play a key role in delivering high-quality, safe and effective medicines optimisation services to our patients.
The successful candidate will have excellent communication and interpersonal skills, with the ability to build positive and effective relationships with patients, GPs, practice colleagues, community pharmacists and other healthcare professionals.
You will be organised, proactive and able to work independently, while also being a strong team player. You will be confident managing a varied workload, balancing competing priorities and using your clinical expertise to support safe prescribing, improve patient outcomes and contribute to the delivery of our clinical and quality objectives.

Main duties of the job
Working within your professional competence and scope of practice, you will support high-quality patient care through medicines optimisation, safe prescribing and effective medicines management.
You will undertake patient-facing structured medication reviews, particularly for patients with long-term conditions, polypharmacy, frailty and complex needs. You will identify medicines-related risks, optimise treatment and support patients to achieve the best outcomes.
You will work collaboratively with GP practices, community pharmacy, secondary care and other healthcare partners to ensure safe transfer of care, including medicines reconciliation following hospital discharge. You will provide pharmaceutical advice to patients and clinicians and support shared decision-making.
You will contribute to local and national medicines optimisation priorities, including prescribing audits, quality improvement, medication safety, antimicrobial stewardship and QOF & PCN objectives. You will promote safe, effective and cost-efficient prescribing while maintaining patient safety and clinical effectiveness.

Person Specification
Experience Desirable
Experience of working in primary care, however full training will be provided. Newly Qualified Pharmacists are also encouraged to apply.
Qualifications Essential
Registered with the General Pharmaceutical Council (GPhC) Degree in Pharmacy (MPharm) or equivalent Specialist knowledge acquired through postgraduate diploma-level study or equivalent training/experience
Desirable
Membership of the Royal Pharmaceutical Society (RPS) Member of, or working towards, Faculty Membership of the Royal Pharmaceutical Society Independent Prescriber qualification or working towards / committed to obtaining qualification CPPE Primary Care Pathway completed or working towards completion
