In this role you will manage a caseload of packages to support individuals with learning disabilities, autism and mental health needs. In this role, you will: Undertake initial holistic nursing assessments and develop individualised care plans in partnership with the client, their family and the care team. Effectively communicate with the care team on the clinical condition or diagnosis and treatment or discharge plans of clients, ensuring accurate records are maintained.
