Job summary
Highparks Medical Practice is seeking an enthusiastic, motivated and patient-focused Clinical Pharmacist to join our established multidisciplinary primary care team on a full-time, permanent basis.
This is an exciting opportunity for a Clinical Pharmacist who is keen to develop their career within general practice and play a key role in delivering high-quality, safe and effective medicines optimisation across our patient population.
The successful candidate will work as an integral member of our multidisciplinary team alongside GPs, nurses, paramedics, healthcare assistants, pharmacy colleagues and other allied health professionals. You will have the opportunity to develop your clinical skills, take responsibility for defined areas of practice and contribute to the ongoing development of our clinical pharmacy service.
Highparks Medical Practice is a well-established, semi-rural practice serving. We operate across our main site at Cliffe Woods and branch sites at Higham, Cliffe and Wainscott. We provide a broad range of general medical services, including long-term condition management and preventative healthcare. We are also actively involved in our Primary Care Network and wider multidisciplinary working.
Applications are welcomed from experienced primary care pharmacists as well as pharmacists who are looking to develop their career within general practice.
Main duties of the job
The successful candidate will work as part of the multidisciplinary team in a patient-facing clinical role, using their expert knowledge of medicines to assess, advise and manage patients safely and effectively.
Key responsibilities will include:
Undertaking comprehensive clinical medication reviews and structured medication reviews.Supporting patients with the management of long-term conditions.Reviewing and optimising medication for patients with complex and multiple morbidities.Identifying and managing medication-related risks and opportunities to improve patient safety.Supporting appropriate prescribing and medicines optimisation in line with national and local guidance.Responding to medication queries from patients, GPs, nurses and other healthcare professionals.Reviewing repeat prescribing processes and supporting safe, efficient and cost-effective prescribing.Supporting medicines reconciliation following hospital discharge and other transitions of care.Providing appropriate clinical advice to patients regarding their medicines, including adherence, side effects and treatment options.Supporting the management of patients with conditions such as hypertension, cardiovascular disease, diabetes, asthma and COPD, where appropriate to competence.Contributing to quality improvement programmes and relevant QOF activity.Supporting the practice in meeting local and national prescribing priorities.Contributing to clinical audits, service evaluation and quality improvement projects.

The Clinical Pharmacist will work as an integral member of the multidisciplinary team, working alongside GPs, nurses, paramedics, healthcare assistants, pharmacy colleagues and other healthcare professionals. The postholder will provide expert clinical pharmacy input to improve the quality, safety and effectiveness of medicines use across the practice.
The role will involve working independently within professional competence, while recognising when referral or advice from other clinicians is required.
Person Specification
Qualifications Essential
Essential Current registration with the General Pharmaceutical Council (GPhC). Demonstrable commitment to continuing professional development. Knowledge and understanding of clinical pharmacy practice and medicines optimisation. Knowledge and understanding of long-term condition management.
