Job summary
We are looking for an enthusiastic, compassionate and organised Care Navigator to join our primary care team.
As a Care Navigator, you will play an important role in supporting patients to access the right services and care at the right time. You will work closely with the wider practice team, helping patients navigate health, wellbeing and community services while providing a welcoming and supportive experience.
About the role
25 hours per week 4 days per week, Monday to Friday Working hours are flexible and allocated by rota between 7:45am and 6:45pm Supporting patients to access appropriate health, social and community services Working collaboratively with GPs, clinicians and the wider primary care team Providing information, signposting and support to patients Helping to improve patient access and experience About you
We are looking for someone who:
Has excellent communication and interpersonal skills Is compassionate, approachable and patient-focused Is organised and able to manage a varied workload Can work confidently as part of a multidisciplinary team Is comfortable supporting people with a range of needs Has a positive, proactive and flexible approach
Previous experience in primary care, health, social care, community services or a similar role would be desirable.

Greeting our patients when they arrive and assisting with enquiries and check insDealing with our telephone lines to assist our patients with queries and appointment bookingsManaging our new Rapid Health safe triage system by care navigating patients to the most appropriate appointment or service.Managing emails and website enquiriesProcessing prescription requestsDaily administrative tasks

Experience Essential
Working within a GP Surgery is preferable and confident on the telephone are pre requisites.
