Bewicke Medical Centre is seeking an enthusiastic and experienced Clinical Pharmacist Independent Prescriber to join our friendly and supportive practice team.
This is an exciting opportunity for a highly motivated pharmacist who can work independently while collaborating effectively with GPs, nurses and the wider team. The successful candidate will play a key role in medicines optimisation, long-term condition management, shared care prescribing and delivery of the new Medicines LES.
Main duties of the job
Undertake structured medication reviews and medicines optimisation clinics. Manage and prescribe within agreed clinical competencies as an Independent Prescriber. Lead on Shared Care prescribing and monitoring, including ADHD, rheumatology and other amber medicines. Support implementation of the NENC Medicines LES, including: Shared care prescribing and monitoring. Injectable medicines services. Oral antipsychotic monitoring. Reconcile medicines following hospital discharge. Support patients with polypharmacy, frailty and complex long-term conditions. Respond to medication queries from patients and practice staff. Review pathology results related to medicines monitoring. Support prescribing audits, quality improvement projects and medicines safety alerts. Work closely with community pharmacy, secondary care and other healthcare providers to ensure seamless patient care. Provide education and prescribing support to clinical staff

Job PurposeWe are seeking an experienced and motivated Clinical Pharmacist Independent Prescriber to join our multidisciplinary team.
The successful candidate will work independently whilst having the support of an experienced clinical team. They will lead on medicines optimisation, structured medication reviews, prescribing safety and management of patients with long-term conditions. The post-holder will play a key role in supporting the delivery of the NENC Medicines Local Enhanced Service, including shared care prescribing, injectable medicines services and oral antipsychotic monitoring.
Main Duties and ResponsibilitiesMedicines OptimisationUndertake structured medication reviews and clinical medication reviews.Manage complex polypharmacy and high-risk medicines.Support prescribing safety and medicines optimisation initiatives.Review pathology results related to medicines monitoring.Reconcile medicines following hospital discharge.Respond to medicines-related queries from patients and clinicians.Support implementation of NICE guidance and local prescribing policies.Independent PrescribingAssess, diagnose and prescribe within scope of competence.Manage patients with long-term conditions through face-to-face, telephone and digital consultations.Support the management of patients with complex medication needs.Work collaboratively with GPs and the wider clinical team to improve patient outcomes.Shared Care and LES DeliveryLead on Shared Care prescribing and monitoring in line with NENC and NTAG guidance.Manage patient recalls and monitoring requirements for shared care medicines.Support existing ADHD shared care patients where prescribing responsibility has been accepted by the practice.Maintain accurate coding, registers and documentation required for LES monitoring and reporting.Support delivery of injectable medicines services and annual oral antipsychotic reviews.Quality ImprovementUndertake prescribing audits and quality improvement projects.Support the implementation of MHRA safety alerts and prescribing guidance.Identify opportunities to improve prescribing quality, safety and cost-effectiveness.Contribute to practice and PCN medicines optimisation initiatives.Education and Team SupportAct as a source of expert medicines advice for the practice team.Provide education and prescribing support to GPs, nurses and other healthcare professionals.Support the development of safe and effective prescribing across the practice.

Registered Pharmacist with the General Pharmaceutical Council Qualified Independent Prescriber. Experience working in General Practice, Primary Care Network or equivalent primary care setting. Strong clinical knowledge of medicines optimisation and long-term condition management. Experience undertaking medication reviews and managing high-risk medicines. Excellent communication and interpersonal skills. Ability to work autonomously and effectively manage workload. Strong organisational and problem-solving skills. Commitment to delivering high-quality patient-centred care.
