About this role

Job summary

We are seeking an experienced and highly motivated Advanced Paramedic Practitioner (APP) to join our multidisciplinary team at Farnham Park Health Group. The role will focus on providing high-quality patient care by supporting clinical staff in managing acute and chronic conditions, undertaking home visits, and contributing to Quality and Outcomes Framework (QOF) achievement work. The successful candidate will play a key role in reviewing blood test results, prescribing medications within their competency, and supporting administrative functions to ensure effective patient management.

-Be the first point of contact for patients presenting with undifferentiated, undiagnosed problems. Making use of skills in history taking, physical examination, problem-solving and clinical decision-making to establish a diagnosis and management plan.

-Refer patients to alternative care setting where appropriate.

-Instigate necessary invasive and non-invasive diagnostic tests or investigations and interpret findings/reports.

-Provide patient education on managing health conditions and preventative care.

-Ensure clinical correspondence is appropriately managed.

We have 3 Partners, 7 Salaried GPs, 1 Paramedic Practitioner, Clinical Pharmacist and an excellent multi-skilled practice nurse team. List size 15,500. Rapid Health, EMIS Web, AccuRX & Docman with Workflow Optimisation. We are part of Farnham Integrated Care Services working in collaboration with local practices offering same day access for urgent care along with MSK Practitioners and Home Visiting Services. X-ray, ultrasound and phlebotomy services available within Farnham Centre for Health. We are also a Training Practice. CQC rated 'good'. 6 weeks annual leave pro-rata and competitive remuneration package, dependent on experience.

Job description Job responsibilities

Main Duties:

-Ensure clinical correspondence is appropriately managed.

Reviewing and Actioning Blood Test Results:

-Request pathology and analyse blood test results and discuss management plans with patients.

-Liaise with GPs and other healthcare professionals to ensure appropriate follow-up.

Supporting QOF Achievement Work:

-Contribute to QOF-related consultations via face-to-face and telephone appointments.

-Assist in chronic disease management and maintaining of registers, including monitoring and reviewing patients.

Urgent Care Support & Duty Doctor Assistance:

-Support the Duty Doctor in triaging and managing urgent, same-day patient cases.

-Conduct face-to-face and telephone triage consultations, providing urgent treatment as required.

-Undertake Duty responsibilities.

Prescribing Medications (Within Competency):

-Prescribe medications where appropriate and within the scope of practice.

-Provide medication reviews and adjust treatment plans accordingly.

Collaboration with Multidisciplinary Team:

-Work alongside GPs, nurses, pharmacists, and administrators to enhance patient care.

-Provide advice and guidance to colleagues, supporting best practice within the practice.

Other Responsibilities:

Develop, implement and embed health promotion and wellbeing programmes

Implementation of group-based consultations for long term conditions

Provide advanced, specialist care to patients as required in accordance with clinical based evidence and NICE guidelines

Maintain accurate clinical records in conjunction with extant legislation

Ensure SNOMED read codes are used effectively

Develop, implement and embed specialist patient clinics.

Support the team in dealing with clinical emergencies

Recognise, assess and refer patients presenting with mental health needs

Be an extended and supplementary prescriber, adhering to local guidelines

Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice), reviewing annually as required

Liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients, etc.)

Support the clinical team with safeguarding matters in accordance with local and national policies

Person Specification

Qualifications Essential

-Qualified Paramedic with HCPC registration. -MSc or Postgraduate Diploma in Advanced Clinical Practice (ACP) or equivalent. -Independent Prescriber (V300) qualification. -Experience working in a primary care or urgent care setting. -Strong clinical assessment, diagnostic, and management skills. -Competency in managing chronic conditions and acute presentations. -Excellent communication and interpersonal skills. -Ability to work independently and as part of a multidisciplinary team. -IT proficiency and preferred experience clinical systems such as EMIS.

Desirable

-Experience in Quality and Outcomes Framework (QOF) work. -Experience supporting urgent triage and duty doctor roles. -Previous experience working in General Practice.
