As an LPN or RN, you will play a vital role in delivering person-centered healthcare services, supporting individuals in residential settings, and empowering staff through clinical guidance and training.
This position is ideal for a compassionate nurse who enjoys building relationships, providing hands-on care, and serving as a healthcare resource for individuals, families, and support teams.
What You'll Do
Direct Nursing Care
• Provide nursing services that support each person's individualized goals, health needs, and quality of life.
• Assess, monitor, and document changes in physical or mental health status.
• Administer medications and treatments in accordance with physician orders, the Nursing Practice Act, and Charles Lea Center policies.
• Provide skilled nursing care including:
• Injections (excluding insulin)
• Wound care
• PEG tube feedings
• Tracheostomy care
• Other medically necessary treatments
Care Coordination
• Attend medical appointments and advocate for individuals receiving services.
• Communicate significant health concerns and recommendations to Program Managers, physicians, and interdisciplinary team members.
• Complete health assessments, medication audits, and follow-up reviews.
• Participate in developing and implementing Individual Service Plans and health-related supports.
Training & Leadership
• Train and support direct care staff on:
• Medication administration
• Adaptive equipment and assistive devices
• Health-related procedures and best practices
• Conduct medication pass competency evaluations and refresher training.
• Ensure proper documentation and compliance with applicable regulations and agency policies.
Person-Centered Support
• Provide hands-on assistance when needed, including:
• Meals and feeding support
• Bathing and personal hygiene
• Dressing and grooming
• Mobility and transfers
• Recreational and rehabilitative activities
• Work collaboratively as part of the residential team to ensure individuals receive quality care and support.
On-Call Responsibilities
• Provide on-call nursing support and guidance to residential programs.
• Respond to urgent medical concerns and make on-site visits when necessary.
• Assist with ensuring prescribed medications and treatments are implemented promptly.
Qualifications
Required
• Current South Carolina LPN or RN license in good standing.
• Valid driver's license and reliable transportation.
• Ability to attend appointments and travel between residential locations as needed.
• Strong clinical assessment, documentation, and communication skills.
• Ability to work a flexible schedule and participate in an on-call rotation.
Preferred
• Experience working with individuals with intellectual and developmental disabilities.
• Experience in residential, long-term care, community-based, or behavioral health settings.
• Experience providing staff training and clinical oversight.

Work Environment: Primary duties may be conducted in a remote setting with the flexibility and ability to work at a specific CLC location as/if needed. CLC’s interdepartmental Remote Work Policy will apply. This position involved distribution of medications in a variety of settings which may require frequent walking. This position may also come in contact with patients who have contagious illnesses.
Physical Requirements: This role involves regular standing, walking, lifting (up to 50 lbs), and hands-on support. Reasonable accommodations are available.
Travel: Frequent travel via car within Spartanburg County to homes/apartments and other locations as needed.
