Community Learning Disabilities Nurse - Band 5
We are currently seeking a passionate and motivated Learning Disabilities Nurseto join our community team. This opportunity is ideal for a newly qualified nurselooking to begin their career in a supportive environment, or for an experienced practitionerseeking to further develop their skills in community-based learning disabilities nursing.
You will be joining a well-established and experienced team of Learning Disabilities Nurses who are committed to delivering high-quality, person-centred care. Comprehensive support and mentorship will be provided, along with access to funding for continuing professional developmentto support your growth and career progression.
Following a structured induction period, you will manage a caseload of individuals with a range of healthcare needs. Your role will involve working directly with service users, as well as collaborating with families, carers, and social care professionals to ensure holistic and effective care planning. Key areas of focus will include:

Main duties of the job
To provide specialist support, advice and guidance to individuals, families and carers. To provide specialist support as part of the multi-disciplinary team To complete specialist assessments as clinically indicated To participate in PBS training and offer specialist support within the PBS framework To participate in referral meeting, team meetings and wider MDT meetings. To contribute to team and service development. To carry an individual caseload. To keep up-to-date with research and development in your fields of practice To attend training relevant to the role To maintain NMC registration and remain up-to-date with research and development in learning disability nursing practice To complete individual risk assessments under supervision and to put a safety plan in place if needed. To communicate with social workers, care providers and other agencies and services. To supervise and assess student nurses on placement with the team.
