Job summary

We are looking for an enthusiastic and committed 4 session salaried GP to join our friendly and supportive team.

We pride ourselves on providing high-quality, patient-centred care while supporting our clinicians. You will work alongside an experienced team of GPs, ANPs, practice nurse, pharmacists, dispensers, healthcare professionals and an excellent admin support team

Main duties of the job

The successful candidate will provide high-quality, patient-centred care through a range of clinical and administrative responsibilities. This will include face-to-face and telephone consultations, responding to patient queries, undertaking home visits and nursing home visits, reviewing and acting on investigation results and correspondence, and completing clinical administration, including referrals. The GP will use professional judgement to make safe and autonomous clinical decisions, while working collaboratively as part of a supportive multidisciplinary team.

Hodnet Medical Centre is a rural dispensing practice with a list size of 4200. Situated in the historic village of Hodnet, North Shropshire, 15 miles north of Shrewsbury. With good transport links to Birmingham and Manchester.

Being a small practice with a stable patient population we get to know our patients, often many generations of the same family. Job description Job responsibilities

In accordance with the practice timetable, as agreed, the post-holder will make themselves available to undertake a variety of duties including:

Face to face and telephone consultations and queries,

Visiting patients at home and in nursing homes

Checking and authorising prescriptions

Reviewing clinical results and correspondence in a timely fashion.

Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation

Assessing the health care needs of patients with undifferentiated and undiagnosed problems

Screening patients for disease risk factors and early signs of illness

Developing care plans for health in consultation with patients and in line with current practice disease management protocols

Providing counselling and health education

Recording clear consultation notes to agreed standards

Recording health data correctly to meet QOF and enhanced service specifications

Collecting data for audit purposes

Compiling and issuing acute and repeat prescriptions.

Prescribing in accordance with the practice prescribing formulary (or generically) whenever this is clinically appropriate

In general, the post-holder will be expected to undertake all the normal duties and responsibilities associated with a salaried GP working within primary care.
