Join our Community Nursing Rapid Response Team as a Lead Practitioner.
An exciting opportunity has arisen for an experienced and motivated clinician to provide strong clinical leadership, supporting the delivery of safe, effective, and patient-centred care within a busy 24-hour service.
Applicants must hold current NMC registration, together with Physical/Health Assessment and V300 Independent Prescribing qualifications.
Working alongside the Operational Team and senior nursing colleagues, you will support day-to-day caseload management, risk management, governance, reporting, and service improvement initiatives. You will also play a key role in developing the Lead Practitioner role and shaping future service delivery.
We are looking for a skilled practitioner with excellent communication, negotiation, and interpersonal skills, who is confident in challenging practice appropriately and supporting the development of others. You will demonstrate competence in core community nursing skills and a commitment to continuous learning.
The role is primarily Monday to Friday, 9.00am to 5.00pm, although flexibility is required to provide occasional support outside these hours to meet the needs of the 24-hour service.

As Lead Practitioner, you will utilise your enhanced assessment skills to support decision-making and be competent and confident across areas of practice and provideenhanced holistic nursing care to housebound service users utilising enhanced assessment skills; decision making; prescribing and freedom to act as a Lead Practitioner.
As part of this role, you will be expected to:
Manage and co-ordinate a caseload of service users with complex physical health needs that require multi-systems health assessment; have multiple co-morbidities and may have social and mental health needs that influence their treatment plans. This may at times include co-ordination of MDT meetings. Take a lead in specialist areas and complex caseload management, providing advice and support to others to undertake community nursing care. Formulate plans of care and negotiate the implementation of such plans and the sharing of complex, sensitive, confidential, and at times contentious information e.g. explaining diagnosis and treatment plans; discussing disease progression and prognosis. Support staff development in the service by providing mentorship and supervision to pre and post registered nurses.
