Main duties of the job
Based within the Quality Improvement Team (QIT), you will play a pivotal role in helping the Practice deliver high quality, outcome-focused care. You will take ownership of allocated patient recall pathways and lead or support a range of clinics, including PSA monitoring, Diabetes Year of Care reviews, Severe Mental Illness (SMI) health checks, as well as contributing to joint heart failure review clinics.
This role offers a true blend of clinical and non-clinical responsibilities. Alongside data-led recall and quality improvement activity, you will deliver hands-on HCA-led care, carrying out elements of Diabetes Year of Care reviews and SMI physical health checks in line with national guidance and practice protocols.
You will also be an integral part of the teams ad-hoc clinics, which are developed in response to ongoing reviews of QOF targets and performance. Working closely with GPs, nurses, and the wider multidisciplinary team, you will help ensure timely patient reviews, accurate data capture, and effective engagement.
Through proactive collaboration and intelligent use of practice and patient-level data, youll help improve outcomes for people living with long-term conditions while supporting the Practice to achieve QOF and locally commissioned services and meet its contractual and performance objectives.

The primary goal of this role is to collaborate closely with the Quality Improvement Team (QIT) to evaluate, enhance, and continuously monitor the Practice's systems. This is aimed at optimising revenue generation and improving patient outcomes. By leveraging both Practice and patient data, the role seeks to boost overall performance, enabling the Practice to achieve its objectives and enhance healthcare delivery to its patient base. Additionally, the role entails conducting specialised clinics focused on the management of Long-Term Conditions (LTCs), emphasising comprehensive and effective patient care.

Person Specification
Qualifications Essential
Care Certificate Minimum requirement of GCSEs A-D including a C in English or equivalent.
Knowledge and Skills Essential
Knowledge of general office procedures The ability to respond to appropriate requests for assistance, maintaining confidentiality whenever necessary Be able to work under pressure and have clear systems or strategies in order to prioritise workload The ability to be patient, caring, calm and empathetic
Desirable
Knowledge of EMIS Working knowledge of chronic disease or conditions such as heart disease, COPD, diabetes and their management Awareness of co-morbidities in Long Term Conditions
Experience Essential
Previous experience of working in general practice Experience of working in a busy office environment IT literacy including excellent proficiency in word processing, excel and data entry Experience of managing and prioritising multiple tasks Experience in working as a member of a team
Desirable
Experience/competence in: dementia screening, blood pressure checks and venepuncture
