About this role

Job summary

We are looking for an experienced and enthusiastic Advanced Nurse Practitioner to join our friendly, supportive and forward-thinking GP practice.

This is a full-time role working 37.5 hours across 5 days, providing consistent ANP cover throughout the working week. You will work as part of a well-established multidisciplinary team, managing patients with acute and minor illness, assessing undifferentiated presentations, prescribing where appropriate and supporting the delivery of high-quality same-day care.

We are a busy practice, but one with a strong team ethos and a real focus on providing accessible, personalised care for our patients. You will have the autonomy to manage your own clinical workload while having the support of an experienced GP, nursing and wider practice team.

Main duties of the job

Assess, diagnose and manage patients presenting with acute and minor illness. Independently manage undifferentiated clinical presentations. Prescribe medication within scope of practice. Request and interpret appropriate investigations. Provide treatment, advice, onward referral and safety-netting. Work closely with GPs, nurses, pharmacists and the wider multidisciplinary team. Maintain accurate clinical records and work in accordance with practice policies and clinical guidelines. Support the delivery of safe, effective and responsive General Practice services.

Job description Job responsibilities

The Advanced Nurse Practitioner will:

Assess patients presenting with acute, minor and undifferentiated illness. Take comprehensive clinical histories and undertake appropriate physical examinations. Formulate differential diagnoses and develop appropriate management plans. Prescribe medication safely and appropriately as an Independent Prescriber. Request and interpret investigations including blood tests, imaging and other diagnostic tests where clinically indicated. Initiate treatment and provide appropriate follow-up and safety-netting. Refer patients to secondary care and other services when required. Recognise and appropriately manage deteriorating or acutely unwell patients. Provide health promotion, prevention and self-care advice. Work within current evidence-based guidance and local and national clinical pathways. Maintain clear, accurate and contemporaneous clinical records. Communicate effectively with patients, relatives, carers and other healthcare professionals. Work closely with GPs, nurses, pharmacists and the wider multidisciplinary team. Participate in clinical meetings, significant event reviews, audits and quality-improvement work. Contribute to the ongoing development of clinical services within the practice. Maintain professional registration, mandatory training and continuing professional development.

Clinical Practice

The postholder will be expected to exercise a high level of professional autonomy and clinical judgement while recognising the limits of their own competence and seeking advice or onward referral when appropriate.

They will manage their own clinical workload while working as part of a supportive multidisciplinary team with access to GP advice when required.

Patient safety, effective communication and high-quality personalised care will be central to the role.

Work in accordance with the NMC Code and relevant professional standards. Maintain appropriate professional indemnity and registration. Work within their scope of competence and the policies and procedures of St Andrews Medical Centre. Maintain patient confidentiality and comply with information governance and data protection requirements. Participate in clinical supervision and professional development. Identify learning needs and maintain an appropriate CPD portfolio. Contribute positively to a culture of learning, openness and continuous improvement.

Person Specification

Experience Essential

* Experienced in assessing and managing acute and undifferentiated presentations. * Experienced in General Practice, Primary Care or a comparable autonomous clinical environment. * Confident working independently and making clinical decisions. * Able to recognise clinical risk and escalate appropriately. * An effective communicator with a patient-centred approach. * Organised and able to manage a busy clinical workload. * Comfortable working collaboratively as part of a multidisciplinary team. * Committed to high standards of patient care, clinical governance and professional development.

Qualifications Essential

We are looking for someone who is: * An experienced Advanced Nurse Practitioner with relevant postgraduate advanced practice qualifications. * A registered nurse with current NMC registration. * An Independent Prescriber.
