Job summary
Ongar Health Centre is looking for a motivated and experienced clinical pharmacist to join our friendly and supportive team on a full-time basis.
Main duties of the job
Deliver high-quality clinical pharmacy services to patientsUndertake structured medication reviews and medicines reconciliationAct as an independent prescriber to manage and optimise patient treatment plansSupport safe, cost-effective prescribing in line with local and national guidanceManage long-term conditions and contribute to chronic disease managementRespond to medication queries from patients and clinical staff Work collaboratively with GPs, nurses, and the wider practice teamSupport quality improvement and prescribing audit initiatives

Person Specification
Qualifications Essential
GPhC registered pharmacist Hold the GPhC independent prescribing qualification
Experience Essential
An appreciation of the nature of GPs and general practice An appreciation of the nature of primary care prescribing Experience and awareness of common acute and chronic conditions that are likely to be seen in general practice Excellent communication skills with a clear and polite telephone manner Work in a polite, confidential, and discreet manner, respecting patients at all times Confident to work as part of a team and be able to think on your feet independently Strong IT and administration skills Previous experience of SystmOne clinical system is desirable but not essential as training will be given
