Practice Nurse Opportunity

We are a friendly, patient-focused GP practice serving approximately 5,300 patients across our main surgery and branch site. We are looking for a dedicated and compassionate Practice Nurse to join our team and help deliver high-quality care to our local community.

As the sole nurse based at your branch surgery, you will enjoy a varied and autonomous role, building strong relationships with patients and making a real difference to their health and wellbeing. While you will work independently day-to-day, you will have the support of an experienced nursing colleague at our sister branch, providing opportunities for collaboration, advice, and professional support.

Main duties of the job

The successful candidate will provide a high standard of nursing care to our patient population, supporting the management of long-term conditions such as diabetes, asthma, COPD, and hypertension. The role will include undertaking NHS Health Checks, performing cervical smear tests, administering childhood immunisations and adult vaccination programmes, including seasonal flu and COVID-19 vaccinations, and delivering wound care and dressings. The Practice Nurse will also play an important role in promoting health and wellbeing, supporting infection prevention and control procedures, maintaining accurate clinical records, and contributing to the delivery of safe, effective, and patient-centred care.

Job description Job responsibilities

Job Purpose:

Undertake a range of nursing assessments and provide appropriate care / treatment in conjunction with GPs according to practice policy, protocols etc.

Improve standards of care and be involved in identifying practice population needs.

Undertake chronic disease management as required by the Practice.

Knowledge of the GMS contract, e.g. The Quality and Outcomes Framework, LES/DES is a vital element of the post.

Act as Infection control Lead for the practice

Main Tasks Duties and Responsibilities Professional

Make professionally autonomous decisions and provide safe, evidence-based, cost effective, individualised patient care

Deliver person-centred and compassionate care, including support for self-management and behavioural change and motivational interviewing

Attend and contribute to relevant internal and external meetings as required including practice clinical meetings, nurse team meetings and palliative/anticipatory care planning meetings

Maintain a personal progress and training plan to keep up to date with current thinking and developments, as well as identifying own training requirements and attending appropriate courses

Monitor the effectiveness of their own clinical practice through quality assurance strategies such as the use of peer audit, review and appraisal

Maintain professional registration

Work within the latest NMC Code of Professional Conduct

Record accurate and timely consultation data in patients records in accordance with NMC guidance and other standards. Ensure that all data entered is correctly coded

Work collaboratively with colleagues both internally and externally

Maintain membership of an appropriate medical indemnity provider

Work effectively with individuals in other agencies to meet patients needs

Main Tasks Duties and Responsibilities Clinical Role

Monitor, interpret and evaluate patient test results and initiate appropriate treatment, or refer on if required

Make appropriate referrals to primary, community and secondary health care teams

Systematically manage chronic disease in the patient population, including asthma, diabetes and COPD

Undertake opportunistic screening when clinically required e.g. lipids, blood pressure, cervical smears, etc.

Perform venepuncture and electrocardiography

Undertake wound care / management, dressings and suture removal

Undertake leg ulcer management, dopplers and compression bandaging

Carry out ear examination and micro suction

Perform cervical screening and promote breast cancer awareness

Provide an adult and paediatric immunisation service under individualised prescriptions or Patient Group Directions, ensuring safe storage, rotation and disposal of vaccines and drugs

Provide other injections as required and within competence, both IM and SC

Provide advice on lifestyle issues, assist and support patients in changing their behaviours; refer appropriately.

Deliver the smoking cessation service

Deliver travel health care, including vaccinations and travel advice

Recognise the signs of child abuse, family violence, vulnerable adults, substance abuse, addictive behaviours, suicidal tendencies and be aware of local referral procedures

Apply infection control measures when collecting and handling of laboratory specimens, disposing of waste materials, dealing with blood and body fluids spillages and report and treat sharps injuries

Undertake first aid and management of emergencies, e.g. burns, haemorrhage, hypo/hyperglycaemic emergencies/asthma attacks

Undertake annual training and demonstrate competency in anaphylaxis and resuscitation techniques

Provide contraception and family planning advice, including pill checks

Undertake new patient and NHS health checks

Support the practice in achieving quality indicators and targets of the GMS contract and any other relevant service frameworks

Main Tasks Duties and Responsibilities Administration, Audit Quality & Education

Complete all mandatory training modules as specified by the Practice

Contribute to research and development programmes where required to improve clinical practice

Participate in practice audits as required

Assist in seasonal and special projects as requested e.g. flu campaign

Restock and maintain clinical areas

Carry out regular checks on emergency equipment and schedule maintenance as required

Ensure safe storage, rotation and disposal of vaccines and drugs; ensure all drugs are in-date and specified stock levels are maintained

Carry out drug and vaccine stock take and re-ordering on monthly basis under supervision

Maintain health promotion literature and information for patients

Apply infection control measures within the practice according to local and national guidelines, including:

Hygiene precautions

Collection and handling of laboratory specimens

Segregation and disposal of waste materials

Decontamination of instruments and clinical equipment

Reporting and treatment of sharps injuries

Dealing with blood and body fluid spillages

Periodic audits/inspections

Work safely at all times in accordance with Legislative requirements and Practice Policy and Procedures
