Job summary An exciting opportunity has arisen for an experienced nurse to pilot and lead on the development of our new PCN and Neighbourhood Frailty Team. Experience of long term condition management with the associated post graduate diploma qualifications, leadership and line management of registered professionals are essential to deliver on our ambitious goals. Initially, you will lead a team of healthcare assistants to support the collation of clinical data required to reviews long term conditions for housebound and care home patients. Completing desk top reviews, with a requirement to carry out home and care home visits as required for patient care and clinical supervision. This is a key role in leading the expansion of this service to further realise our neighbourhood ambitions.
