Job summary
Please note: at this stage, the role is open only to colleagues currently employed by the Kent and Medway Integrated Care Board (ICB) and Southeast Integrated Care Boards; and current at-risk employees within Kent and Medway NHS Trusts.
The postholder is responsible for delivering individual transformation, commissioning and service improvement projects within the Multi Neighbourhood portfolio across Kent and Medway. They will support the development and delivery of integrated, community-based models of care, ensuring alignment with ICB priorities, national policy, neighbourhood health approaches and wider system transformation ambitions. The role will lead defined areas of strategy, needs assessment, service redesign, commissioning policy and planning, ensuring that services are evidence-based, outcome-focused, financially sustainable and responsive to population need.
The postholder will support PSR-compliant commissioning, contribute to planning and assurance cycles, maintain effective strategic relationships with partners and work flexibly across the wider team to support collective delivery of transformation priorities.
Main duties of the job
Lead defined Multi Neighbourhood transformation projects and workstreams, ensuring alignment with ICB priorities, national strategy, neighbourhood health approaches and wider system transformation plans. Support the redesign and development of integrated community-based models of care that improve outcomes, reduce variation and support care closer to home. Develop commissioning approaches, strategies, needs assessments, commissioning intentions and service models that enable transformation across the Multi Neighbourhood portfolio. Use population health intelligence, service performance data, financial information and stakeholder insight to identify opportunities for improvement, transformation and service sustainability. Work collaboratively with NHS providers, local authorities, VCSE partners, neighbourhood teams and clinical colleagues to support integrated pathways and improved continuity of care. Retain responsibility for relevant Multi Neighbourhood priorities, including dementia, statutory duties and delegated functions, ensuring these are delivered within a transformation and improvement framework. Contribute to PSR-compliant commissioning, value for money, contract management and provider performance, ensuring these support delivery of agreed transformation outcomes. Support assurance, reporting and governance arrangements, providing clear updates on progress, risks, issues, benefits and outcomes.

Person Specification
Qualifications and Experience Essential
Experience of transformation, service redesign, improvement methodologies and integrated care delivery Experience supporting development of strategies, needs assessments or commissioning policies. Educated to degree level or able to demonstrate equivalent experience in commissioning, health management or a related field Experience managing budgets, contracts or obligations, ensuring PSR compliance and value for money Experience working with NHS providers, local authorities, VCSE partners or neighbourhood teams Experience analysing performance data and applying findings to commissioning decisions.
Attributes Essential
Ability to analyse complex information and develop clear, evidence based recommendations. Skills in commissioning or analytical software.
