Job summary Promoting effective self-management of patients and/or providing professional intervention for community management in order to present unnecessary hospital admissions, improve quality of life and to actively identify patients who would benefit from case management. 2. This role will include leading the organisation and management of the service and resources, caseload management, benchmarking, clinical audit, the provision of respiratory education and the development of teamwork by identification of common and individual aims. 3. The post holder will make a contribution to both patient and professional education related to the management of respiratory disease in Waltham Forest. 4. The post holder will provide advice and information for the development of the respiratory service in Waltham Forest 5. The post holder will hold caseload and carry out home visits or clinics as appropriate
Main duties of the job
To undertake the comprehensive assessment of patients with respiratory disease , particularly Asthma patients including those with complex presentation and those reaching end of life, using investigative and analytical skills, and to formulate individualised management and treatment plans, using clinical reasoning and utilising a wide range of treatment skills and options to formulate a specialised programme of care. 2. To undertake a holistic, unified assessment process in partnership with all agencies, to ensure that acute admission is prevented where possible and that this approach identifies and delivers services to meet and sustain patients needs. 3. To interpret and analyse clinical and non-clinical facts to form accurate diagnosis and prognosis for patients with complex respiratory disease, to recommend the best course of intervention and to develop comprehensive management plans. 4. To monitor patients with Respiratory Disease, evaluate and modify treatment in order to measure progress and ensure effectiveness of intervention working with the appropriate health and social care professional, ensuring that a seamless approach to care is maintained across all agencies 5. To keep accurate up to date records of patients defined within the case finding process and communicate timely and informative written reports and summaries for other agencies e.g. GPs, Consultants, MDTs and Social Services.

To establish robust communication networks with the patients, their carers other members of the multi-disciplinary team as well as other agencies, and the whole health economy involved in the care of the patients. 7. To provide spontaneous and planned advice, teaching and instruction to patients and/or relatives, carers and other health and social care professionals to promote an understanding of the aims of any given intervention and to ensure a consistent approach to patient care.
The ability to travel effectively across the boroughs is necessary, including home visits
Main duties of the job
1. To further develop Waltham Forest Respiratory services in partnership with Key Stakeholders.
2. To provide high-quality leadership qualities for the Respiratory team by having and implementing organisational knowledge relating to trust protocols and procedures and supporting all to adhere to them.
3. Support the development of high - quality respiratory care throughout the respiratory care pathways, supporting others to do the same through integrated and collaborative working.
4. The respiratory care/education will be delivered in local NHS or community venues, in the patient’s own home or residence, as individual or group sessions as appropriate.
5. To be responsible for own caseload management
6. To participate in the trusts appraisal and supervision system and support others to do the same.
7. To keep accurate and up to date patient records in accordance with local trust, professional body, and legal requirements
8. To participate in and ensure effective risk management behaviours across the WF Respiratory Service by accident/ incident reporting, assessing and controlling risk and ensuring residual risks are added to the trust register.
9. To line manage, supervise and appraise junior members of staff and support the development of staff under direct supervision.
Detailed job description and main responsibilities
The post holder will 1. Be a Registered clinician on the NMC register and hold a first Degree or equivalent experience. 2. Have a teaching or mentoring qualification or experience. 3. Have evidence of ongoing personal/professional development in Respiratory at degree level or above. 4. Be a non-medical Prescriber (or with a commitment to working towards this) 5. Be responsible for providing, supporting and developing the delivery of high quality; evidence based respiratory clinical practice throughout the local community. 6. Be responsible for the delivery and further development of the WF Respiratory Service including the delivery of Education programmes for people living with respiratory conditions. 7. Further develop education programmes for the population across WF supporting patients (carers and their families) to actively participate in self- management of their respiratory conditions to optimise their own health and well-being. 8. Promote independence through high quality and effective leadership; the post holder will demonstrate active leadership skills in their work with the WF Respiratory Service Team members, that embraces effective team-work through identification of common and individual aims for the service. 9. Will support the development of high quality care throughout the Respiratory pathways supporting others to do the same. 10. Will promote effective self-management of patients to aim to prevent unnecessary hospital admissions and improve quality of life for people living with ll be a source of and provide respiratory information and education for NELFT community services to follow this ethos 11. Will have a commitment to collaborative integrated working and actively support and liaise with other health professionals and agencies. 12. This role will include assisting in the organisation and management of local respiratory services which will include evaluating local service provision to inform resource and caseload management, the undertaking of benchmarking exercise against national guidelines and NELFT/service clinical audit

Qualifications Essential
NVQ level 2 or equivalent standard of literacy and numeracy
Evidence of continuous professional development
RN with current NMC registration status
Degree in Physiotherapy or Adult Nursing

Experience Essential
To have a minimum of 3 years experience working as a band 6 RGN , with complex patients.
Previous experience working within the speciality managing patients with Asthma/COPD
Experience of working in a multidisciplinary environment with a wide range of individuals at different levels.

Manage staff conflict and difficult situations
