Job summary
The Dynamic Care Team project support officer provides high-quality administrative and coordination support to the Care (Education) and Treatment Review (CTR/CETR) and Dynamic Support Register (DSR) processes. The post holder plays a key role in ensuring that records, meetings, reporting and communications relating to people with a Learning Disability and/or Autism who are at risk of admission, or in hospital, are managed accurately, safely and in line with National guidance, local policy and statutory requirements. The role supports integrated working across health, social care, commissioners and provider organisations and also providing general administration support to senior leaders and teams, as required.
Main duties of the job
CTR / CETR Administrationo Coordinate and administer CTR and CETR meetings, including:o Scheduling meetings and securing appropriate attendeeso Preparing and circulating agendas, papers and reportso Maintain accurate records of all CTR activity in line with NHS England Assuring Transformation requirements.o Support the preparation and submission of CTR data returns and reports.Dynamic Support Register (DSR)o Maintain the local Dynamic Support Register, ensuring data is:o Accurate, up to date and quality checkedo Managed in accordance with information governance and confidentiality requirementso Support weekly DSR review meetings, including documentation, minutes and action tracking.o Work with partners to ensure timely updates relating to risk status, admissions, discharges and care planning.Data, Reporting and Qualityo Produce routine and ad hoc reports to support service oversight, assurance and performance monitoring.o Support data validation exercises and respond to data quality queries.o Contribute to audit and assurance processes linked to CTR and DSR activity.Communication and Coordinationo Act as a key point of contact for CTR and DSR related enquiries.o Liaise professionally with a wide range of stakeholders including:o Clinical teamso Local authoritieso NHS Englando Independent reviewerso Ensure sensitive information is always handled appropriately.

General Administration Support the effective implementation of key projects by providing high quality administration, co-ordination and project support to deliver priorities and improve outcomes for the population of Leeds. Have lead responsibility for the design and delivery of specific administrative tasks and processes including monitoring of generic inboxes and the organisation, management and action log management of meetings and boards. May also act as a key point of contact. Undertake specific tasks on behalf of business units and matrix teams, including the organisation of meetings involving stakeholders from across the city and production of documents, eg slide decks. Use agreed methods, approaches and documentation to ensure a high quality and systematic approach to project specification, planning, dependency mapping, risk management, stakeholder management, highlight and exception reporting. Undertake the administration and support delivery of specific improvement methods and PHM approaches to enable delivery of key priority areas within matrix teams. Support more senior programme managers by delivering effective administration support to enable the successful delivery of more complex change programmes and projects being delivered across the organisation and the Leeds Health and Care Partnership.GeneralThe post holder will: Treat people with dignity and respect. Be experienced in complex and detailed minute taking Experience of using Data Base systems and the full suite of Microsoft Office packages ECDL, RSA3 or equivalent computer and IT skills including accurate and advanced keyboard skills and experience
