Job summary
Would you be interested in joining our friendly team of multidisciplinary healthcare professionals? With the creation of 2020/21 Network Direct Enhanced Service (DES) and the introduction of the NHS Additional Role Reimbursement scheme for new primary care roles, an opportunity has arisen for Primary Care Networks (PCNs) to bring in roles that support GP practices, Networks and elements of the Network DES.
This role will contribute to tackling healthy lifestyle issues in Shrewsbury and surrounding areas, predominantly via the delivery of the NHS Health Check Programme. The NHS Health Checks will be delivered to individuals across the Practice population in a variety of settings, offering tailored advice and encouragement to make behavioural changes accordingly, as well as onward referral to other relevant services.
Main duties of the job
The post holder may work with seldom heard communities who experience inequalities in health, such as asylum seekers, the homeless, ethnic minority groups and those living in areas of high deprivation. They will have responsibility for reaching out to, and providing lifestyle input into this cohort of patients. They will work closely with the other members of the primary care team and complex care teams in the management and decision making about care and service provision for individual patients. This will include:
Engage with and invite individuals for the NHS Health Check programme, to the programme, offering a quality service that provides tailored advice and encouragement to make behavioural changes accordingly.Care planning, health coaching and delivery of systematic self-management support based on knowledge of individual's activation levels.Support for individuals to access appropriate community resources and services.

Qualifications & Experience Essential
Educated to GCSE level or equivalent. Evidence of working within a team setting. Experience of working in a role that requires a high degree of autonomy.
Desirable
NVQ Level III (health and social care) or equivalent. Formal training in working with long term conditions. Experience of delivering NHS health checks. Experience of working within a multidisciplinary team. Experience of working in primary care.
Personal Attributes Essential
High level of written and verbal communication skills and the ability to build rapport and establish good one to one relationships quickly. Ability to actively listen, empathise and provide personalised support in a non-judgmental way, to inspire trust and confidence. Ability to organise, plan and prioritise under own initiative. Ability to multi-task. Reliable, punctual and confident.

Ability to deal with challenging behaviours.
Knowledge & Skills Essential
Evidence of ability to work both autonomously and seek support and guidance when appropriate. Knowledge of and ability to work to policies and procedures including confidentiality, safeguarding, information governance and health and safety. IT skills including use of email communication and generic software packages such as Microsoft Word, Excel and PowerPoint.
Desirable
Knowledge of Primary Care and how it operates. Knowledge of Primary Care Networks and the Network Contract DES specifications. Understanding of healthy and unhealthy lifestyles and the broader health issues affecting the population including nutrition, smoking cessation, physical activity and alcohol. Knowledge and understanding of the local community, including local services and how others might use them.
Other Essential
Meet DBS standards and criminal records check. Willingness to work flexible hours when required to meet work demands. Access to own transport / full clean driving licence. Ability to travel across the locality on a regular basis.
