About this role

Job summary

The Group Clinic Facilitator coordinates group based clinical sessions for an invited group of patients, primarily those with Chronic conditions. This role supports shared medical appointments by encouraging participant engagement and fostering a safe and inclusive environment for all those attending. The facilitator works as part of a team to improve patient self management, access to care, clinical outcomes, and patient experience. They actively participate in review of each session, providing feedback to enable the continuous evaluation of the Group Clinic model.

Main duties of the job

Assist clinicians with pre clinic planning and preparation of all clinical information for each facilitator session Facilitate structured group clinic sessions according to established criteria and program guidelines Provide a welcoming introduction to the group clinic session and ensure confidentiality forms have been completed at the start of each session Guide initial discussion to explore patient led questions to their condition Create a safe, supportive, and inclusive environment Encourage peer learning and social support among group participants Monitor group dynamics and, with the clinician, manage any challenging behaviour in a professional manner Escalate any safeguarding concerns, help to identify barriers to care Support with the education of participants Facilitate the end of the session, ensuring participation feedback is received and there are aware of any clinical follow up planned Participate in evaluation of each group clinic in liaison with the leading clinician Provide feedback Provide Administration support to the Group Clinic Co ordinator as required to ensure effective planning and organisation of sessions, thus, providing a positive experience for both patients and healthcare professionals. Support the practice to continue to develop the Group Clinic model and promote its benefits to colleagues and patients.

The Adam Practice is an innovative, large 5 site practice, caring for approximately 40,000 patients. Our team of around 170 people includes many allied health professionals, including doctors, paramedics, nurses, healthcare assistants, support, and administration staff.

The group facilitator will be required to undertake related programme training and in house shadowing in a supported environment.

Participation in an annual individual performance review, including taking responsibility for maintaining a record of own personal and or professional development

Taking responsibility for own development, learning and performance and demonstrating skills and activities to others who are undertaking similar work.

The post-holder will strive to maintain quality within the practice, and will:

Alert other team members to issues of quality and risk

Assess own performance and take accountability for own actions, either directly or under supervision

Contribute to the effectiveness of the team by reflecting on own and team activities and making suggestions on ways to improve and enhance the teams performance

Work effectively with individuals in other agencies to meet patients needs

Effectively manage own time, workload and resources.

The post-holder should recognize the importance of effective communication within the team and will strive to:

Communicate effectively with patients and carers

Recognise peoples needs for alternative methods of communication and respond accordingly

Contribution to the Implementation of Services:

The post-holder will:

Discuss with other members of the team how the policies, standards and guidelines will affect own work

Participate in audit where appropriate.

Actively take part in mandatory and internal and external training as requested.

Be aware of government guidelines in relation to confidentiality, clinical information etc.

Adhere to and follow all policies and protocols as listed in the Staff Handbook and contract and apply practice policies, standards and guidance

Follow all Practice Health and Safety guidelines and policies and inform the H&S lead in the surgery of any infringements.

Person Specification

Technical Skills Competencies Essential

Basic level of literacy and good IT skills Ability to work under pressure and be flexible to change dependant on the situation.

Qualifications Essential

GCSE grade A to C in English and Maths

Skills & Attributes Essential

Excellent communication and interpersonal skills, including the art of active listening. Good organisational skills and timekeeping skills Ability to use own initiative and be adaptable Able to communicate with people at all levels and maintain a neutral stance to encourage an open conversation. Ability to handle sensitive information confidentially Ability to self-motivate, organise and prioritise workload Flexible approach and willingness to adjust to the changing needs of the practice

Experience Essential

Experience working within healthcare or other coaching services. Experience of working directly with people in a supportive manner. Ability to work as part of a team.

Desirable

Experience and working knowledge of primary care
