This unique position offers you the opportunity to work as part of a highly skilled community workforce who are the cornerstone of safe patient care. You will work collaboratively with the multi-disciplinary team to visit patients in their own homes and care homes to avoid unnecessary hospital admissions. We have lots of training and development pathways at every level through from band 3 to band 8. We provide a preceptorship program to support you in your role and ongoing training and development.
We use electronic clinical records through EMIS and a work IPad and IPhone will be provided.
Main duties of the job
We are seeking a highly motivated and ambitious Senior Registered Nurse to join the wider Community Nursing Team which includes Community Matrons, District Nurses, Registered Nurses, Nursing Associates and Healthcare Assistants. You will work under the direction of a District Nurse to assess, implement and evaluate person centred care for a caseload of patients. You will undertake clinical skills including venepuncture, wound care, catheterisation and end of life including syringe drivers.
Post holders will be available to work Monday to Sunday between 8am and 8pm. A full UK driving license and vehicle for business use if required for this post, however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.

Communication
Communicates effectively with patients, carers, and all members of the multidisciplinary team.
Maintains accurate and legible documentation and clinical records in line with professional and organisational requirements this includes contemporary paper records and electronic clinical record system, EMIS.
Acts as the nominated Lead Nurse and oversees daily handover ensuring record of the daily handover is logged.
Maintains strong working relationships and communicates effectively with GPs, practice nurses, social services, acute / community hospital colleagues, borough council staff and the local voluntary sector.
Communicates effectively with others involved in patient care including weekends / out of hours, using mobile phone, and emails to enable a timely response to patients needs.
Attends GP/caseload meetings and MDT meetings if the District Nurse caseload holder is unable to.
Actively contributes to locality based, team, working groups and GP meetings.
Patient Care / Safety / Quality Works under the direction of the District Nurse to document and ensure high quality and effective assessment, treatment, implementation, and evaluation of nursing care to patients and carers within their own home setting.
Takes responsibility for a defined caseload in the absence of the District Nurse.
Responsible for the effective triaging of referrals and allocation of work across a locality within the Community Nursing Team.
Prioritises daily work in line with clinical / patient need and escalates where there are issues with undertaking daily visits.
Undertakes full nursing assessments of patients, including those with multiple pathology and complex health and social needs.
Responsible for agreeing the patients care plan, liaising with others, and adapting the care and treatment
delivered according to need and patient choice.
Responsible for the monitoring of medication regimes, assessing effects and advising patients on the safe storage and disposal of drugs.
Initiates, implements, and evaluates health promotion activities and self-care strategies with individuals, patients, and carers.
Evaluates care and treatment in agreement with the patient and as defined in the care plan.
Escalates any deviation from the care plan or change in condition at hand over and to the District Nurse / GP accordingly.
Responsible for the correct use of aids and equipment supplied for patients and carers and for the security, care and maintenance of equipment ensuring standards of infection control and safety are maintained including equipment loaned to patients.

Acts as a link nurse for defined area e.g. tissue viability, end of life, diabetes, infection prevention and control.
Leadership and Management Support the District Nurse with operationally managing Registered Nurses in the community nursing team.
To monitor stock levels of dressings and other equipment in line with budget and to be responsible for the ordering of dressings and other equipment in line with lean stock management and efficient principles.
Ensures that all complaints, untoward incidents, accidents, and areas of clinical risk are identified and reported to the District Nurse and on the electronic incident reporting system (DATIX) in accordance with policy
Responsible for the security, care and maintenance of equipment ensuring standards of infection prevention and control and safety are maintained

Qualifications Essential
Registered Level 1 Nurse with current Nursing and Midwifery Council registration Educated to level 6 / degree level in relevant area Assessor Supervisor / Mentorship qualification
Knowledge and Experience Essential
Broad range of clinical skills relevant to Community Nursing that are actively undertaking, - including but not limited to Compression Bandaging, Dopplers, Venepuncture, Male & Suprapubic catheter, Syringe drivers and Palliative care Symptom Management. Ability to delegate nursing care and to prioritise visits in accordance with patient need and to organise workload without direct supervision A full UK driving license and vehicle for business use if required for this post, however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010
