About this role

Job summary

This vacancy has created the opportunity within the Hospital Outreach 24/7 Team for a dynamic and enthusiastic individual to join the Senior Enhanced Practitioner Team and wider Team to support the implementation of Martha' Rule across the organisation to ensure that patients and families have a clear accessible route to escalate concerns.

The post holder will work as part of the Hospital Outreach 24/7 Team and will respond quickly and effectively to deteriorating patients on the wards. They will work alongside Multi-Disciplinary Teams (MDTs) using the Prevention, Identification, Escalation and Response (PIER) Model to reducing physical deterioration. They will also have responsibility for the daily management of a caseload of patients, in collaboration with the medical teams.

This post will be split between Worcestershire Royal Hospital, Worcester and The Alexandra Hospital, Redditch and travel across the county is essential.

Main duties of the job

Work towards health promotion and prevention and comprehensively assess patients for risk factors and early signs of illness and deterioration.

Draw on a diverse range of knowledge in their decision-making to determine evidence-based therapeutic interventions, which will include prescribing medication where legally allowed and actively monitoring the effectiveness of therapeutic interventions.

Plan and manage complete episodes of care, working in partnership with others, delegating and referring as appropriate to optimise health outcomes and resource use, and provide direct support to patients and clients.

Work in partnership with patients and support them to manage and live their life.

Person Specification

Qualifications Essential

Registered Healthcare Practitioner; Teaching/Assessing Course; Intermediate life support; Completed a Level 6 or 7 Health Assessment module; Competent in venepuncture, cannulation, arterial blood gases, NG Tube and Line Management; Independent Prescriber

Desirable

MSc in Advanced Clinical Practice; Advanced Life Support

Knowledge Essential

Understanding of clinical governance and risk assessment; Up to date knowledge of evidence-based guidelines and policies

Other Job Requirements Essential

High level of self-motivation, drive for performance and improvement; Flexibility in approach and attitude; Shows honesty and integrity; Able to work cross site and full rotation across 24-hour period and 7 days (if service requires)

Knowledge Essential

Understanding of clinical governance and risk assessment; Up to date knowledge of evidence-based guidelines and policies

Experience Essential

Relevant post-registration experience, within speciality; Able to demonstrate recent effective clinical leadership skills; Participation in audit and clinical research
