About this role

Job summary

The post-holder will be managed by the ward manager F3, and to ensure the patient's journey through the hospital is as smooth as possible without any unnecessary delays. Most importantly, administrating and managing patients' discharge. This role is front of house which requires the post holder to maintain a professional and courteous manner when dealing with colleagues and members of the public. The role of Care Co-Ordinator fulfils a link between Consultants, Medical Teams, Ward Nursing Staff, Social Care Services, external agencies patients and their relatives.

Main duties of the job

The role is an essential support to the relationship between Wards, Clinical Site and patient flow team. This is by communicating the immediate position on the bed occupancy and discharge information on the ward. Furthermore, the care co-ordinator will support the operational management team by providing information and expertise on patients' discharge and delivering on the operational priorities for that.

Person Specification

Education and qualifications Essential

Achievement through study to level 3 or above (NVQ, GCSE A-C or equivalent, 1st Diploma, Apprenticeship. Level 2 Math's & English qualification or equivalent

Experience and knowledge Essential

Computer literacy Computer/word processing skills Ability to work on own initiative and within a team Understands the importance of patient confidentiality

Skills and abilities Essential
