About this role

Job summary

We are looking for an experienced and enthusiastic Practice Nurse to join our friendly team at Kings Medical Centre on a 9-month fixed-term contract to cover maternity leave, starting in January 2027.

There may also be an opportunity for a permanent position at the end of the maternity cover period, subject to the needs of the practice at that time.

We know that flexibility is important, so we are happy to discuss hours and working days with the right candidate.

You will be joining an established nursing team which includes a Lead Practice Nurse and three Healthcare Assistants, working alongside our wider clinical and administrative teams.

Job description Job responsibilities Long-Term Condition Management

Undertake reviews and support the ongoing management of patients with long-term conditions, in accordance with individual competencies and practice protocols. This may include:

DiabetesAsthmaCOPDCardiovascular diseaseHypertensionOther long-term conditions appropriate to the post holder's training and experienceSupporting the practice in achieving QOF and other relevant clinical targetsTreatment Room DutiesProvide a range of Practice Nursing services, which may include:

Wound assessment, management and dressingsRemoval of sutures and clipsCervical screeningContraception reviews, where appropriately trainedBlood pressure monitoringUrinalysis and specimen collectionInjections and medicines administrationECGs, where requiredHealth promotion and lifestyle adviceOther routine Practice Nursing procedures within the post holder's competenceAnticoagulation / INR MonitoringUndertake INR testing and anticoagulation monitoring for patients prescribed warfarinUse INRstar to record results and support anticoagulation management in accordance with practice protocols and agreed clinical pathwaysProvide patients with appropriate advice regarding their anticoagulation treatmentIdentify and escalate abnormal or concerning results appropriatelyMaintain accurate and up-to-date anticoagulation recordsWork within own level of competency and relevant anticoagulation protocols at all timesImmunisations and VaccinationsAdminister childhood immunisations in accordance with the national immunisation programmeAdminister adult vaccinations as appropriateSupport and participate in the practice's seasonal influenza and other vaccination programmesEnsure vaccines are administered safely and appropriately under relevant Patient Group Directions (PGDs), Patient Specific Directions (PSDs) or other appropriate legal mechanismsMaintain accurate vaccination recordsFollow cold-chain procedures and report any concerns appropriatelyHealth Promotion and PreventionProvide patients with appropriate health promotion and lifestyle adviceSupport screening and prevention programmesIdentify opportunities to promote healthy lifestyles during patient contactsSupport patients to make informed decisions about their health and careClinical ResponsibilitiesThe post holder will:

Work closely with the Lead Practice Nurse and three Healthcare AssistantsCommunicate effectively with GPs, nurses, HCAs, pharmacists, administrative and reception staffSupport colleagues and contribute to a positive working environmentAttend relevant practice, nursing and training meetings where requiredShare knowledge and experience with colleaguesRaise concerns appropriately and contribute to identifying solutionsSupport new members of staff where appropriateWe believe that good patient care starts with a team that works well together. The post holder will therefore be expected to support the practice's focus on teamwork, communication and staff wellbeing.

Professional DevelopmentThe post holder will be responsible for:

Person Specification

Qualifications Essential

Registered Nurse with current NMC registration Evidence of relevant continuing professional development Appropriate training and competency in core Practice Nursing duties Cervical screening qualification/training Immunisation and vaccination training, including childhood immunisations

Desirable

Long-term condition management qualifications or recognised training Anticoagulation / INR monitoring training Contraception and sexual health training Relevant Practice Nursing qualification or diploma

Experience Essential

Previous experience working as a Practice Nurse within General Practice Experience of managing and reviewing patients with long-term conditions Experience of cervical screening Experience of delivering childhood and adult immunisations Experience of wound care and dressings Experience of health promotion and preventative care Experience of working independently within own scope of practice Experience of working effectively as part of a multidisciplinary team Experience of maintaining accurate electronic patient records
