Nottingham CityCares Community Public Health 019 Nursing Service is looking for a motivated and enthusiastic Community Public Health Nurse to join our 519 School Health Team based at Wollaton Vale Health Centre.
The ideal candidate will have:
Excellent communication skills, both written and verbal A positive, flexible and proactive approach A genuine interest in working with children, young people and families A commitment to improving health outcomes and reducing inequalities Good IT, organisational and prioritisation skills The ability to work both independently and as part of a team Knowledge and experience of safeguarding children and young people An understanding of key public health issues affecting school-aged children and young people in Nottingham City

We support staff with continued professional development and work in partnership with Derby University to host Specialist Community Public Health Nurse students for School Nursing. Many of our Community Public Health Nurses have progressed into Specialist roles within the service.
Working days to be discussed at interview. The service operates Monday to Friday, 8:00am to 5:00pm, excluding weekends and bank holidays.

Main duties of the job
You will be part of Nottingham CityCares Childrens Public Health 019 Nursing Service, working within the 519 School Health Team based at Wollaton Vale Health Centre.
The role will involve supporting the Specialist Community Public Health Nurse School Nurse in delivering services for school-aged children, young people and their families across Nottingham City.
You will contribute to the delivery of the Healthy Child Programme through health promotion, early intervention, targeted support and safeguarding. Working closely with schools, families and partner agencies, you will help identify health needs, support children and young people to access the right help at the right time,and contribute to reducing health inequalities.

Job Purpose
The aim of The Childrens Public Health 0-19 Nursing Service is to reduce health inequalities and improve the health and wellbeing of children and their families in Nottingham City. The team offers a universal service for all children and young people in Nottingham City as set out in the Healthy Child Programme.
The role of the Community Public Health Nurse is:
To work within a multi-disciplinary team, supporting the Specialist Community Public Health Nurse School Nurse (SN)/ in the delivery of the universal and targeted services in a variety of community settings including citywide working. To support the SN in the management of a defined caseload, as part of an 5-19 years integrated multi-agency partnership approach. Working to promote health and contribute to tackling health inequalities in line with local and national guidelines, public health priorities and Safeguarding responsibilities. To work as part of a multi-disciplinary team sometimes without direct supervision but reporting back to the SN on a regular basis. To work with our most vulnerable children within Nottingham City with the expectation to attend safeguarding meetings to include ICPC, RCPC, Core Groups, LACS, ERMM, Strategy Meetings, liaising with the caseload holder. To use a child & family approach to working, in a solution focused way. To promote holistic healthcare to children, young people, and families and to work in partnership with other agencies. To lead on specific programmes of care for children and young people who have specific/complex needs. To lead on and participate in health promotion initiatives e.g., childhood immunisations, in line with local and national guidelines, public health priorities and safeguarding responsibilities. To support the development of the care pathways required for this population group to reach their optimum potential. To provide advice, signposting and support for parents, carers and young people using Text Health, our text messaging service. To ensure that the rights, needs and dignities of children, young people, and families are met by providing health promotion and appropriate programmes of care. To promote the health of children, young people to enable each child to reach his/her full development and educational potential which supports the philosophy of inclusive education. To work in an inner city with a rich and varied ethnic, cultural, and socioeconomic climate.

Key Responsibilities
Clinical
To follow a competency-based induction and training pathway supported by your mentor. To support the Children and the Community Public Health Nursing Team to deliver in Healthy Child Programme 5-19 years. This will include the National Childhood Measurement Programme. To undertake home visiting programmes to offer support to carers/parents in the care of their children following competency-based training supervision. To work in a variety of settings including clients homes, Childrens Centres, health premises and education settings across Nottingham City. Following allocation of appropriate work relating to children, young people, and families by the SN the post holder will have responsibility for the assessment; development; implementation and evaluation of programmes of care for individual children and those children with complex health care needs and their families Ability to organise and prioritise own workload under the supervision of senior members of the team. You will be able to use appropriate evidenced based tools to assess the physical and emotional wellbeing of children, young people, and families To deliver evidence-based programmes of care adhering to standard operating procedures and pathways of care Deliver drop-in sessions. To work within CityCare policy and procedure, in line with your NMC code of conduct and NICE Guidance. Demonstrate good communication, initiative, organisational and interpersonal skills and be able to work flexibly in a team setting & Citywide as the need arises. Attend team / allocation meetings. Appropriately delegate work to skill mix team. Provide education on local and national targeted public health initiatives through individual and group sessions. Provide and receive routine information, orally, in writing or electronically in line with Nottingham CityCare policy and abiding by legal requirements. Ensure clear, accurate and up to date recording of information on SystmOne as per local record keeping policy. Maintain an accurate up to date diary in accordance with lone working policy and adhere to mobile working guidance. To work in partnership with children, young people, and families and with local partners across the Nottingham Childrens Partnership, statutory and voluntary agencies To work with families identifying and taking appropriate action / make appropriate referrals in any situation where there may be vulnerability, safeguarding or health concerns. To access and participate in safeguarding supervision. To prepare reports, under supervision, for child protection, children in need, Looked after Children, Common Assessment Framework and Priority Families. Take the lead professional role and attend meetings and reviews where appropriate. To conduct oneself in a corporate and professional manner always maintaining confidentiality including compliance with the CityCare dress code policy. To attend essential, mandatory, and appropriate training courses and meetings, thus keeping professional development current. Ensure practice is evidence based to maintain high quality, effective public health care. To demonstrate leadership and contribute to the appraisal, development, and monitoring of performance of team members induct and mentor staff and students. To undertake mentorship, preceptorship, and clinical supervision as per policy.
