Job summary
Glenpark & Teams Medical Services has an exciting opportunity for an enthusiastic, experienced Nurse Practitioner to join our friendly and committed nursing team.
The position is for 37.5 hpw working across both of our sites in Dunston and Teams.
You will be joining a great team lead by an experienced ANP, working alongside 2 Practice Nurses, 2 Nurse Associates, a Healthcare Assistant and 2 Phlebotomists. Regular mentoring sessions will be provided, along with support for both career and personal development within the role/team.
Applicants should be confident in managing patients with single and multiple Long-Term Conditions and have experience in managing urgent on the day medical problems. The successful candidate must be confident managing patients in a primary care setting and be able to demonstrate NMC requirements for the role. They must be eager to work as part of a team and to provide person centred care to our patients. We encourage professional development and offer clinical supervision on a weekly basis, along with regular practice educational meetings.
We would welcome visits to discuss the role further and for you to meet the team. We would also be interested to hear of any skills you would wish to develop or bring to the team.

About us
We are a GMS partnership with two practice sites, serving a list of approx. 15,000 patients. Glenpark Medical Practice and Teams Medical Practice are both well established, high-achieving practices and are part of Gateshead Inner West PCN and consistently maintain a high QOF achievement.
The practice comprises 6 GP Partners and 8 salaried GPs, supported by the Practice Managers/Assistant Practice Manager and a highly skilled and responsive reception and administrative team. We have an established nursing team led by an Advanced Nurse Practitioner and consists of 2 Practice Nurses, 2 Nurse Associates, a Health Care Assistant and 2 Phlebotomists.
As a practice, we support the nursing team throughout their professional development as well as also being a training practice for Doctors in Training and medical students. The practice holds a strong belief in the importance of continuous professional development as well as personal wellbeing. The practice also specialises in substance misuse, provides a minor surgery service and LARC service.
We work closely with our PCN team comprising Pharmacists, Pharmacy Technicians, Physiotherapists, Mental Health Nurse and Social Prescribing Link Workers.
We have close working relationships with our community colleagues and have an active Patient Participation Group.

To manage daily surgeries including face-to-face and telephone consultations. This may also include some visiting of patients either in their own home or in a care home environmentTo manage patients presenting with a range of acute and chronic medical conditionsTo collaborate with GPs, nursing staff and non-clinical support staffTo request, interpret and manage pathology and radiology test results, as well as referring patients or request specialist /secondary care advice when appropriate.Maintain accurate and timely clinical records, ensuring SNOMED codes are used effectively, maintaining chronic disease registers.Support the clinical team in dealing with medical emergencies and remain up to date with Basic Life Support/ Anaphylaxis management skillsTo assess and refer patients presenting with mental health needs as appropriateTo provide education, mentoring and training to other staff and students as appropriateBe an independent and supplementary nurse prescriber, adhering to guidance and support patients in the use of over the counter or prescribed medicinesContribute to practice targets (QOF, enhanced services) and follow relevant local and national guidelines for management of acute and chronic conditionsDelegate clinical responsibilities appropriatelySupport the clinical team with all safeguarding mattersParticipate in practice significant event review processes and meetingsKeep professionally updated and be responsible for addressing any unmet personal training needs.
Person Specification
Experience Essential
Long term condition management Management of acute and chronic presentations of illness Paediatric clinical skills/nurse prescribing Experience of the Year of Care approach to Care and Support Planning for Long Term conditions, or an interest in developing this Excellent interpersonal and team-working skills. Excellent time management skills, ability to prioritise and manage own time and work to timeframes.
Desirable
Experience using electronic patient record system, eg EMIS Experience of working in primary care/GP Practice
Qualifications Essential
