Job Summary:
The Behavioral Health Registered Nurse (RN) serves as a core member of the collaborative care team, which includes the patient’s medical provider, counselor, and psychiatric provider. This position promotes patient-centered care through education, coordination, and follow-up for individuals with mental health and substance use needs.
This position will need to support both Geneva, NY and Sodus, NY locations.
Main Responsibilities:
Care Management
• Support the mental and physical health care of patients on an assigned caseload, collaborating with medical and behavioral health providers.
• Facilitate engagement, follow-up, and referrals for patients requiring specialty or higher levels of care.\
• Monitor progress and ensure continuity of care through communication with patients and interdisciplinary teams.
• Participate in case reviews and team consultations to address complex or high-risk patients.
Clinical Services
• Screen and assess patients for common mental health and substance use disorders.
• Provide education on behavioral health conditions, treatment options, and available community resources.
• Deliver brief, evidence-based behavioral interventions such as behavioral activation, problem-solving treatment, or motivational interviewing as appropriate.
• Monitor treatment response and assess for symptom changes, medication side effects, and adherence through in-person or telephonic follow-up.
• Support psychotropic medication management as prescribed by medical providers by monitoring adherence, effectiveness, and side effects.
• Effectively engage patients in therapeutic relationships to support ongoing treatment and recovery.
Medication Assisted Therapy (MAT)
• Serve as the coordinator for the Medication Assisted Therapy (MAT) program, collaborating closely with prescribers and counselors.
• Obtain and observe urine drug screens as required during patient appointments.
• Ensure accurate documentation and continuity of care for all MAT patients.
Medical Record Maintenance
• Chart services in patient record including but not limited to:
• Referrals, medications, vaccinations, injections, screenings, lab orders/results, patient care goals, patient history, vitals
• Audit progress notes to ensure all patient history and vital signs have been taken and noted by nursing staff
• Chart and lock notes within timely manner in accordance with Finger Lakes Community Health policies and procedures
• Address telephone encounters, actions, and referrals in the electronic health record system in accordance with Finger Lakes Community Health policies and procedures
Other Duties
• Perform other related duties as assigned by the supervisor.
Education and Qualifications:
• A current, unrestricted NYS Registered Nurse License is required
• Prior experience in Behavioral Health preferred.
• CPR certification required (can be provided)
• 3+ years in a healthcare or customer service setting, acting as a subject matter expert and operating independently required.
• The ability to work flexible hours -- some evenings are required
• A valid government-issued photo identification document is required
Physical Requirements:
• Sitting/Standing: Prolonged sitting with occasional standing or walking.
• Computer Use: Frequent typing and prolonged screen time.
• Moderate Lifting: Regularly lifting or carrying items weighing between 15-40 pounds.
• Visual: Clear vision for reading and inspections.
• Hearing Clarity: Ability to hear conversations in person or in virtual communications.
• Noise Sensitivity: Work may require filtering background noise to focus on speech or signals.
• OSHA Level 1
