Responsible for ensuring interdisciplinary pharmaceutical care programs are appropriately integrated throughout the institution. Participate in all aspects of the medication use system including formulary management, clinical surveillance, and clinical policy/protocol/guideline development and implementation. Provide comprehensive pharmaceutical care in assigned areas. Champion and operationalize quality and regulatory compliance initiatives designed to improve medication use and pharmacy practice. Assist in the continuous oversight and growth of the residency program. Provide medication and practice related training and education to pharmacy team, members of the patient care team, and patients.

Education/Training: PharmD required. PGY1 in applicable specialties strongly preferred. Ability to read, write and communicate in English.

Experience: Greater than 3 years of hospital experience as a clinical pharmacist/clinical pharmacy specialist/clinical coordinator/advanced practice pharmacist preferred. Clinical experience with antimicrobial stewardship, anticoagulation stewardship, and/or opioid stewardship preferred. Residency Program management experience preferred.

Licensure, Registration or Certification: Current New York State pharmacist license/ registration renewal required. Pharmacotherapy or other relevant Board Certification preferred or willing to obtain within 1 year of eligibility.
