Job summary Coordinate and support high-quality, person-centred care for residents across 11 nursing and residential care homes and a respite care facility. Conduct monthly visits to care homes to review resident needs, monitor care plans, and support proactive care management. Undertake weekly visits to the respite care home to support reviews, care coordination, and communication with care staff. Liaise with the practice, care homes, residents, families, and external healthcare providers to ensure continuity of care. Support clinical assessments, patient reviews, care planning, and follow-up actions. Coordinate referrals, appointments, investigations, and healthcare interventions. Maintain accurate patient records and documentation in line with practice policies. Monitor residents requiring enhanced support and escalate concerns appropriately.
